Through the promulgation of Law No.

17/2023 on Health (“Law 17/2023”), matters pertaining to the


issuance of Registration Certificates (Surat Tanda Registrasi – “STR”) and Practice Permits (Surat Izin
Praktik – “SIP”) have now been fundamentally altered. However, as the implementing regulation
that will specifically address these matters has yet to be enacted, the Minister of Health (“ Minister”)
has issued Circular No. HK.02.01/MENKES/1911/2023 (“Circular 2/2023”), which should be used as a
stopgap reference during the issuance of STR and SIP in the wake of the issuance of Law 17/2023.

This Circular 2/2023 directly affects all medical workers and healthcare personnel who require
licenses to be issued/renewed, given that possession of STR and SIP is necessary for said personnel
to engage in medical practice.

Issuance of STR

In line with the transitional provisions set under Law 17/2023, Circular 2/2023 states that any STR,
Temporary STR and Conditional STR that were issued and that were still valid prior to the
promulgation of Law 17/2023 will remain valid until the end of their validity periods. Furthermore,
before or after the validity periods of the abovementioned STR expire, Circular 2/2023 also allows
for medical workers or healthcare personnel to renew their STR so that they subsequently have
lifelong validity through the submission of applications to the Indonesian Medical Council along with
their previous STR. This represents a significant revision, as under the now obsolete framework of
Law No. 36 of 2014 on Healthcare Workers, STR only had five-year validity before re-registrations
were required.

It is important to note that any STR that were issued prior to the promulgation of Law 17/2023 but
its validity period has expired for less than three months since the submission of the relevant
renewal applications, the requirements that apply for renewals of STR into Lifelong STR are the same
as the those outlined above (specifically the submission of an application with the previous STR
attached). However, in terms of STR that were issued prior to the promulgation of Law 17/2023 but
for which the validity periods has expired for more than three months since the submission of the
relevant renewal applications, the following requirements now apply instead:

1. Medical workers or healthcare personnel who fulfilled the Professional Credit Unit (Satuan
Kredit Profesi – “SKP”) requirements during the five-year period prior to the promulgation of
law 17/2023 will be allowed to apply for renewals into Lifelong STR through the submission
of applications which should enclose a diploma and/or professional certificate and a
certificate of SKP fulfillment; or
2. Medical workers or healthcare personnel who have yet to fulfill the SKP requirements
must first complete a competence test organized by the Ministry of Health and must
afterward submit their STR renewal applications along with a diploma and/or professional
certificate and a certificate of SKP fulfillment.

In addition to the STR renewal process, Circular 2/2023 also sets out the procedure for the issuance
of new Lifelong STR. Similar to the renewal process summarized above, applications for new Lifelong
STR should be submitted to the Indonesian Medical Council. However, the following
documents/items must be enclosed with all such applications:
Finally, it should also be noted that all of the procedures outlined above should be conducted
through the STR Online Registration application, which has been integrated into the SATUSEHAT
application.

Issuance of SIP

In a manner similar to the provision of STR, Circular 2/2023 states that any SIP that were issued prior
to the promulgation of Law 17/2023 will remain valid until their validity periods expire. In addition,
Circular 2/2023 also sets out the procedure for the issuance of new SIP and SIP extensions that
should be followed by medical workers and healthcare personnel.

It is important to note that based on the above-described applications, the Head of the relevant
Regency/City Health Service or Head of the relevant Regency/City Regional Investment and One-
Stop Integrated Services Facility, as authorized officials, should issue SIP that have five-year validity

Circular 2/2023 has been in force since 24 August 2023.
