low vision.

ALBINISM

Child with albinism

History
3-year-old boy, living at home with his mother, father and older
sibling.
Diagnosis:
Oculocutaneous Albinism, Nystagmus, moderate myopic
astigmatism (wears glasses), compensatory head posture with
null point.
Goals identified:
1) Develop fine and gross motor skills
 Improved mobility in unfamiliar environments
 Independent movements in and out of home- riding bike etc.

2) Develop play skills


 Ability to sort, classify and categories objects.

3) Increased independence with daily self-care tasks


 To commence toilet training.
 To be able to participate in helping to dress himself.

4) Develop expressive and receptive language skills


 To become more familiar with positional language.
 To be able to understand and express his emotions.

5) Develop age-appropriate social skills

History
Patient is an 89-year-old male who has presented today
reporting that he has difficulties with reading and personal
tasks.
Medication history
For blood pressure and occasionally arthritis pain

Retinitis pigmentosa
History
A 30-year-old female, diagnosed with RP when 15 years
old with gradually reducing vision over time.
Chief Complaint
She is unable to see at all at night now. She has previously
not been aware of what might have been available to her
and rarely went for ophthalmological review due to being
scared of the outcome.
Examination
She has just booked an ophthalmological review and
visual field test with the ophthalmologist she had first seen
when 15yo.
