Mayflower St., Mandaluyong City, referred to in this Agreement as ACTIVEONI -and- David Saguil, M.D. Filipino, of legal age, a Doctor of Medicine duly licensed to practice under Philippine laws, with address at 41 Juan Luna st. Area 1, UP Campus, Quezon City, referred to in this Agreement as the CONSULTANT; ACTIVEONE and CONSULTANT are collectively referred to in this Agreement as “Parties”. THE PREMISES: 1, ACTIVEONE maintains on-site medical clinics for its clients. 2. The professional services and expertise of duly licensed physicians are needed in said on-site medical clinics. 3, The CONSULTANT is qualified, available, and willing to render professional medical services at ACTIVEONE’s on-site medical clinics, as specified under this Agreement.
