Quezon City., after having been duly sworn to in accordance with law, hereby depose and state that;

1. I am the one to file to Siniloan, Laguna Registry of Deeds.

2. Authority given by the registered owner’s SPA, attached.
3. It was discovered that TCT T-4701, technical Description was erroneously written.
4. Certified Technical Description of the lot from Land Registration Authority (LRA), Dated Sept.
07, 2022, attached the original technical Description.
5. The following is the correct Technical Description of the lot under TCT T 4701;

6. The missing line in TCT’s Technical Descriptions are line 4-5 and point 5-6, in Tct’s Technical
Description from point 4 jumped to point 7, this correction from typical error wanted to
correct.
7. Also, the Registered address in the TCT of the registered Owner from 20 Ferrari St.,Fairview
Park Subd., Quezon City, Metro Manila to no. 30 Don Juan, Don Antonio Hgts., North, Quezon
City, Metro Manila.

thereof in the TCT no. T-4701 by the Register of Deeds Siniloan, Laguna Branch., done
