    INSPECTION FINDING REPORT

    Equipment no RM4 Inspection Date 7/10/2021


    Equipment Description RM4 Gearbox main drive Inspected by Che Dee

    Finding Description
    Abnormal sound was heard on 3.10.2021 from the gearbox. Further inspection on 7.10.2021 in the gearbox
    using endoscope found severe pitting on the gear tooth. From oil analysis report on 20.4.2021. Slightly high
    level of iron noticed.

    1. To continue monitor the vibration trending. 2. To monitor the oil sample analysis for the gearbox. 3. Plan
    for replacement.
