EYFS Nursery On-Entry Activity Booklet Group Record Sheets: Adult Guidance

These record sheets can be used to record the data collected during initial observations for the EYFS Nursery On-Entry Activity Booklet, which can
be found in this pack.

You may find it easiest to print the sheets onto A3 paper. Some pages in this resource use a key linked to the activities in the Activity Booklet, such
as using the letter ‘P’ to show that child was unable to name a shape but pointed to it with adult support.

Key:

P = child needed support to complete the task/activity or is developing the skill mentioned

 = child completed the task or activity
