NURS3137 Mental Health Nursing Clinical Practicum
Clinical Exam

Instruction Guide

1. Each student will be assigned one patient. The student is expected to complete an assessment of their patient. The assessment
should include the data gathering interview (includes the Mental Status Examination - MSE) and a head to toe physical
assessment survey. Additional information may also be needed from the patient’s records.
2. The examiner will observe the physical and mental status assessments (aspects of the MSE, the MSE or the MMSE as
indicated).
3. Student’s will have a maximum of one hour and a half to complete this aspect of their exam.
4. Student’s may be asked questions by the examiners during observation.
5. Students will be allowed to complete the clinical worksheet for a maximum of 30 additional minutes.
6. Students are expected to complete one care plan for the client with two nursing diagnoses including one actual diagnosis and at
least four nursing interventions with rationales. The nursing diagnoses should be numbered according to priority.

NB
Students would have received the clinical examination tool and should be given clear instructions on what is expected at the beginning
of the exam. In keeping with their preparation for the Final Clinical exams, students should complete their exam with minimal
prompting. They will be assessed on what is observed and presented.
