UNDERTAKING / WAIVER

To Whom This May Concern:

level of the Arellano University, do hereby undertake:

1. That I shall submit on or before ____________________ to the Registrar’s

Office the following documents:

[ ] Form 138
[ ] Good Moral Character
[ ] Birth Certificate
[ ] Transcript of Records
[ ] HD / Transfer Credential
[ ] Permit to Cross-Enroll
[ ] _______________ and,

2. That, in case of failure to submit the foregoing document/s on the said date
or such extended date within the sole discretion of the University, my enrollment shall
be deemed cancelled, and I shall obliged (a) to forfeit in favor of the University all
payments already made and to pay the balance of the total tuition and other school
fees; or (b) to forfeit the full payment already made in favor of the University, as the
case maybe.

I clearly understand that on my failure to do submit the above stated

documents, no unit/s shall be credited in my favor for the subject/s I currently enroll.
