UNDERTAKING

I, Joshua V. Baldo, of legal age, Filipino citizen, single/married, and residing at Block
10, Lot 17, Fairgrounds, Vallejo Place, Phase 4, Pasong Buaya 2, Imus, Cavite, commit
and undertake to the following facts and disposition, to wit:

1. That I have applied and been formally approved for admission at the Arellano
University School of Law (AUSL);
2. That I have read and voluntarily assented to the AUSL Policy Guidelines on
Application and Admissions;
3. That I have substantially complied with the submission of documentary requirements
by submitting the following: (mark check or X)

_____ Original/Certified True copy of birth certificate
_____ Original/Certified True copy of official transcript of records (as
required for Arellano Law Copy or for further studies)
_____ Original copy of Honorable Dismissal (when applicable)
_____ Signed Copy of Grades (for transferees for computation of grade weighted
average)
_____ Certificate of Good Moral Character from School where you were graduated
(for freshman)
_____ Certificate of Good Moral Character (for Transferee)

2. Signed by one former professor
_____ Completely accomplished Application Form (in duplicate)
_____ 2 pcs of 2X2 recent colored photo with white background and printed name.

4. That I cannot at this stage submit the above X-marked credentials required by AUSL
for the following reasons beyond my control: I haven’t been able to send a 2X2 photo
for the reason that it wasn’t only recently that I was allowed to go out and have my
picture taken. I will send it immediately. For the PhilSAT Certificate of Eligibility, I
was supposed to take the exam this year, however, the pandemic happened.
5. That I have exerted earnest efforts to prepare the same for submission as evidenced
by the following and photocopy herewith attached:

6. That I promise to submit the aforementioned credentials on or before the end of the
semester that I am going to enroll in;
7. That I submitted a copy of grades previously enrolled in another law school and
formed part of my application package which has not been fraudulently altered; (for
transferees only)
8. That by virtue of my deficiencies, I understand that my enrollment at the AUSL is
CONDITIONAL pending completion of all required documents;
9. That I am fully aware that failure on my part to submit the required documents within
the prescribed period shall result in the DENIAL of my enrollment in the succeeding
semester; and
10. That I, having voluntarily executed this Undertaking in a private document
understand that the stipulation herein are valid and binding between the undersigned
and AUSL, as represented by the Admission and Alumni Affairs Office.
