MEMORANDUM

You are hereby instructed to follow the herein Terms of Reference in the inspection and monitoring
of extracted and transported sand and gravel, boulders, limestone, mountain quarry, phyllite schist
(mangima stones) and other mineral resources; and compliance of administrative concerns in your
respective area of assignment/checkpoints:
1. During inspection, the Quarry Inspector must undertake the following steps:
2. The designated checkers of Barangay LGUs are not allowed to collect pink coupons, only the
BENRO quarry inspectors are legally responsible given that Delivery receipts/Coupons are
accountable forms. In the loss of said document, the Quarry inspectors/Supervisors shall be
charged with simple neglect of Duty which is equivalent to suspension.
3. Type of vehicle used as indicator to transport quarry resources that has an equivalent load
capacity with the coupons issued by the Provincial Administrator’s Office;
a. Sand and gravel, boulder, and other quarry resources aggregates that were already
inspected and deposited in the stockyard, especially in Sugod area and other sites as
recommended and approved by the provincial Governor.
b. Partial transport of paid coupons on limestones/sand and gravel and other mineral
resources by small quarry operators (e.g by sack for limestone or purchased coupon is
3 cu.m and only 1 or 2 cu.m is transported) especially in Lingion and Damay Areas
and other sites as recommended and approved by Provincial Governor.
c. For Government Agencies, Non government Organization and Private Individuals
seeking corporate/permit holders social responsibility as part of the Community
Development and Management Program, provided not for commercial utilization as
recommended and approved by the provincial Governor or his authorized
representative; and
d. Transport pass shall be used for one (1) transport only and should be properly
accounted, controlled and recorded in the record book and ETRACS.
e. During inspection, the quarry inspector shall undertake the following steps
1. Secure two copies of an appropriately filled out transport pass from the Permit
Holder/Operator/Contractor/Truck Driver transporting the inspected and deposited
sand and gravel and other quarry mineral resources from the stockyard and ensure it
is accompanied by a Blue DR Coupon with destination or Note Indicating it is going to
a stockyard.
2. The transport pass and Blue Coupon should be stamped with the word “Checked and
Verified” and at the back of both print your name (Quarry Inspector) and affix your
signature, indicate the date and time inspected note “From Stockyard”, then return 1
copy of the transport pass and the blue DR coupon to the assigned Truck Driver and
retain 1 copy of the transport pass for Bukidnon Environment and Natural Resources
Office’s (BENRO) copy
f. Pursuant to Mines and Geosciences Bureau MMC No. 2019-01, Dispatch Receipt
shall be required from Industrial Sand and Gravel (ISAG) Permit Holders for the
transport of sand and gravel beyond one kilometer distance from the extraction area to
their mineral processing facility. Mineral processing facilities located within one
kilometer distance shall therefore be exempted.
5. Quarry Supervisors, Quarry Monitoring and Quarry Inspectors shall strictly comply to the
avoidance of the following administrative concerns which can be grounds for suspension or
termination of service as provided under R.A. No. 2260, known as the “Civil Service Act of
1959” and R.A. No. 6713 known as the “Code of Conduct and Ethical Standards for Public
Officials and Employees”, and its Implementing Rules and Regulations:
a. Receiving money, gifts/tokens and any forms of bribery;
b. Habitual tardiness and absences; and
c. In case of sudden occurrence of illness and other incidence beyond control
encountered by the Quarry Inspector, proper coordination with Quarry Supervisor and
Head of Mines and Geosciences Management Division of BENRO is advised.
6. All extracted and transported mineral resources including observed violations/incidents must
be properly documented and submit to the Provincial Environment and Natural Resources
Officer (PENRO) of BENRO for appropriate action and report to the undersigned.
7. Quarry Inspectors and Quarry monitoring personnel should refrain from answering any non-
technical/non-operational (schedule of activities, PMRB, TWG, etc.) or Non-general (client or
case specific) inquiries. They are to instruct the inquiring person/corporation to address their
concern to the appropriate Quarry Supervisor or directly to the BENRO.
