Please make sure the following is documented for all r/o stroke and
stroke patients:
 Stroke MPP ordered and initiated
 Antithrombotic given within 24 hrs of arrival.
 Dysphagia Screening
 NIH documented
- At admission, discharge or any change in condition
- 24 HRS after admission order is placed (NOT 24 hrs
after first NIH)
 Vital signs and neuro assessment documented Q4H
(EXACTLY at 0800, 1200, 1600, 2000, 0000, 0400)
 Daily Stroke Education
 CM Discharge Instruction for Stroke (Custom) charted
 If pt is on anticoagulation, please document “Stroke
Prevention using blood thinners” education
 PT/OT/ST ordered
 Lipid profile within 48 hrs
 Statin for LDL >70, or not checked within 48 hrs of
admission
 A.Fib: ensure anticoagulant is ordered
 CM evaluation within 24 hours
Stroke Tips:
- Clinical Practice Guidelines can be found under policies - type in
stroke, Stoke Program Guideline & in the attachments you can
find Clinical Practice Guidelines
- Our PI project is the Clinical Excellence Forms (purple sheets)
- We have a stroke support group for our pts that meets
quarterly
- The bulletin board displays our stroke measurements or metrics
to show how we are doing
- Blood pressure management for ICH or ischemic pts is physician
driven by utilizing the MPP
- TPA is given in 1-3 minutes
- We can find goals for our pts in IPOC
- VTE prophylaxis should be documented by hospital day #2
- An ischemic stroke needs to be prescribed an antithrombotic
therapy at discharge or there should be an exclusion note by
the MD documented

- We are educated during staff meetings & huddles on
all stroke information
- Stoke pts need assessments by PT/OT/ST even if there
are no deficits
- A pt that needs to be transferred due to a bleed, the
pt should be transferred to another hospital in 120
min (door to door)
- If pt has A.Fib/flutter, they need anticoagulation. If
there is documentation just once without proof or
clear elaboration, double check with
neuro/hospitalist to have diagnosis removed.
The Joint Commission Virtual Survey:
Surveyors will perform a virtual survey on Thursday, April
22 to re-certify our campus as a Primary Stroke Center. As
part of the virtual survey, Faraz Javed, MD Chest Pain &
Stroke Program Coordinator, will be rounding the campus
with a laptop and iPad with the camera on at all times. The
surveyor’s job is to determine if our organization is
meeting standards and regulations. Please join us in
welcoming the surveyors virtually and support each other
in your interactions with them. Please reach out if you

Summer Time off Request:
Please contact your CCs and Tiffany so that we can
APPROVE the vacation time before you make any plans.
We can only have 2 RN's or 1 PCA from each shift on
vacation on any given week and still be able to staff the
unit. Once you have received notice that you have been
approved, you can then buy tickets etc. If you need a
refresher on our staffing plan, please see Tiffany’s email on
1/4/21.
