INFORMED CONSENT FORM

We, the researchers from Bachelor of Secondary Education Major in English 4B of the
Dominican
College of Tarlac, are currently conducting an action research entitled, “Addressing Reading
Comprehension Gap through Filipiniana Audiobook-Assisted (FAA) Intervention Strategy
among Grade 7 Students at Sto. Niño Academy of Bamban Inc.” under the supervision of
Mr. Arcangel Q. Bañez, Jr, our instructor in FS 1- Field Study 1.

The purpose of the research is to address the Reading Comprehension Gap through a
Filipiniana Audiobook-Assisted (FAA) Intervention Strategy among Grade 7 Students at
Sto. Niño Academy of Bamban Inc.
In line with this, we are requesting your valuable contribution as one of our respondents. You
have been chosen because you are one of the Grade 7 students of Sto. Niño Academy of
Bamban Inc.
Your participation is voluntary. Your participation may include reading comprehension test in
English. The test is expected to take 10 to 15 minutes of your time. Your involvement is
expected to be valid until the final defense and approval of this study.
Please take note that there are no incentives, and you will not be paid for your participation.
Rest assured that your identity will be kept anonymous, all gathered data is confidential and will
be used for academic/research purposes only. If you have questions concerning the research or

my consent and discontinue participation at any time without penalty or loss of benefits to which
I may otherwise be entitled. Should I select to discontinue participation, any information already
collected will be discarded. A copy of this consent form has been given and explained to me. In
