Greetings! I am Christine B. Soriano, a fourth-year student in the AB Political Science program of the
College of Arts, Sciences, and Education of the University of Santo Tomas-Legazpi. I currently have
a study entitled “The Level of Implementation of the Solo Parent’s Welfare Act of 2000 (R.A. 8972,
as amended) in the City of Tabaco.” The said study aims to analyze the solo parents’ programs under
R.A. 8972 conducted in Tabaco City and their effect and benefit to solo parents.
The questionnaire consists of ____ questions and is designed to be answered by solo parents who are
residents of Tabaco City, Albay in order for me to gather sufficient data to complete my
undergraduate thesis. All responses and information gathered through this form will be kept
anonymous and secured as prescribed by the Data Privacy Act of 2012.
