Reflection

A KLM Boeing 747 attempted to take off from Tenerife's Los Rodeos Airport
without receiving permission from air traffic control, colliding with a Pan Am 747
that was waiting on the runway, killing 583 people. The KLM pilot van Zanten's
decision to take off after making assumptions about communications received
from ATC, believing he had the clearance to do so, was found to be the principal
cause of the tragedy. Although most of the focus after the tragedy was on
standardizing phraseology to improve radio communications, the biggest shift
occurred when the aviation industry formed crew resource management, which
addresses the accident's underlying cause.

No matter how long you've been in the utility industry, you've probably heard
that there are some things you should never do. However, certain rapid and
important results influence human behavior. A favorable outcome, such as
lowering the time required to perform a job, is a strong motivator to continue
utilizing the shortcut. A worker is more likely to use the shortcut if the possible
negative consequences, such as punishment or the chance of an event occurring,
do not equal or outweigh the benefit of saving time. Van Zanten did not wish to
exceed his duty-time limits in the Tenerife airport catastrophe. He felt that flying
would provide an immediate, assured, and beneficial advantage. Van Zanten did
