DIAGNOSIS-MODERATE MR,SEIZURE DISORDER

PERSONAL/ADDL SKILLS: Umesh is a quiet child. He is competent

of doing most of the tasks independently. He is a social child ,he greets
everyone who comes to school and says bye while going home. He
follows every instructions given by the teacher for an activity. He shows
concerns for classmates and participates in helping them in doing
various activities. He helps his mother in doing house hold works.

SOCIAL SKILLS : Umesh is a helpful and co-operative boy .He

showing interest to interact with other students of the class. He takes
care of his classmates. He helps his class teacher in conducting various
activities and daily living activities. He maintains himself very well in
respect of cleanliness and hygiene .He is able to speak Hindi and
Marathi languages.
ACADEMIC SKILLS:
(a) Language: He can write three letter words in English and
Hindi. He also can write names of his family members,days and
months.
(b) Mathematics: Umesh is practicing addition, substraction and
multiplication. He is also practicing addition in calculator .He
can identify currency notes (Rs 5,Rs 10,Rs 20,Rs 50,Rs 100,Rs
200) and coins (1rs,2rs,5rs)
(c) Concept-He is average in language and he has difficulties in
solving math problems

MOTOR SKILLS:
(a) Motor skills : He is very good in FMS and can very well
perform Pre-vocational activities. He is good in painting, paper
folding and paper bag making. He can color within a given
space
(b) Gross motor: Umesh actively participated in Childrens day
celebrations. He also participated in Annual sports day . He is
active in games period at school and plays almost every game

RECREATION :
(a) Indoor: He loves to play ludo board and he can solve simple
puzzles. He is also interested in painting and drawing. He is
good in dancing. He is also taught to do art and craft in class.
(b) Outdoor: He participated in Annual sports day and won
passing ball and blind fold. He participated in weekly sports
 periods which includes Badminton Kho-Kho and musical
chairs etc..

GENERAL REMARKS: Umesh has shown lot of improvements in

school. He is now interact and co-operate with each and every student
and teachers as well. This academic session he participated in Childrens
celebration, Christmas and day Annual sports day .He has shown little
improvement in academics.Now a days he shown more interest in
dancing. He is a child who has got lot of scope for development with co-
operation of teachers and parents.
