Subject: Implementation of revised Forms for Aadhaar enrolment & update for 0-5
and 5-18 age groups- reg.

Corrigendum No. 1/2023

As part of strengthening of Aadhaar enrolment ecosystem, E&U-I Division vide OM

No. HQ-16011/2/2022-EU-I-HQ dated 06.02.2023 had issued separate Forms for Aadhaar
enrolment & update for various age groups.

2. In the Forms for 0-5 and 5-18 age groups, it is mentioned under column no. 5 and 8
that ‘Birth certificate with name as Baby of………… is not allowed’.

3. The matter has been reconsidered and above condition is withdrawn with immediate
effect. The revised enrolment & update Forms for 0-5 and 5-18 age groups are attached
herewith.

4. This is issued with the approval of the Competent Authority.
