REASON(S) FOR MINISTRY DECISION:

You are not eligible for a high protein diet supplement as the diagnosis provided of post-op bariatric surgery
does not meet the legislative criteria for a supplement. A high protein diet supplement can be considered for
people with diagnoses such as Hepatitis B/C, cancer, chronic inflammatory bowel disease and osteoporosis.
Unfortunately, post-op bariatric surgery is not covered by the diet supplement legislation. The attached
legislation provides a more detailed list.

If you are dissatisfied with this decision, you can ask the ministry to reconsider it. You have 20 business days
from the day you were notified of this decision to submit a completed Request for Reconsideration form. If you
do not already have one, you can get this form and all the information we considered to make this decision
from your Employment and Assistance office or by contacting the ministry.

We have enclosed the Reconsideration and Appeals brochure to give you more information.
