1. Misdiagnosis:
- Misdiagnosis occurs when a healthcare provider fails to correctly identify a patient's
medical condition. This can happen due to various reasons, such as misinterpreting test
results, overlooking crucial symptoms, or relying on inaccurate information. Misdiagnosis can
be particularly harmful because it may lead to delayed or inappropriate treatment.
- Consequences: Patients may suffer unnecessary pain, complications, or a worsening of
their condition if the true illness or ailment is not addressed promptly. In severe cases,
misdiagnosis can result in disability or death.

2. Surgical Errors:
- Surgical errors refer to mistakes made during surgical procedures. These errors can vary in
nature and severity but often involve negligence or carelessness in the operating room.
Common surgical errors include operating on the wrong body part, leaving foreign objects
(like surgical instruments or sponges) inside the patient's body, or performing a procedure
without obtaining informed consent.
- Consequences: Surgical errors can lead to serious complications, infections, extended
hospital stays, additional surgeries, and long-term physical and emotional trauma for the
patient. In some cases, surgical errors may be fatal.
