I. SUBJECT

Memorandum of Understanding on the Conduct of Dental Mission

2023.under the Adopt A School Program.

II. PURPOSE

As part of the “Adopt A School Program ”, the school , CABAY

NATIONAL HIGH SCHOOL , has been chosen as the recipient of Dental
Mission by King Solomon Lodge No. 150
The Mission has three phase, pre-implementation, implementation and
post assessment. The recipient of this project are the 150 students from the
different grade level ages 14-45
Consequently, implementation process includes some strategies to make
sure that material and other supplemental activities were prepared and in order
With this, students before the extraction were examined, get and record the
oxygen level and blood pressure to ensure their safety. Interviews and other
related informations wre also recorded

.This will be possible through the School Health Coordinator and School
Personnel with the presence of the Barangay Health Workers . Before the
extraction , learners will present their parents’ permit and they should be
accompanied by their parents or guardian .This will benefits CNHS learners and
parents.

IV. UNDERSTANDINGS

A. Provided Dental Mission to 150 Learners of Cabay National High

School ages 14- 150.
B. There will be an assigned Dentist from King Solomon and Several
personnel to assist the learners with the assistance of CNHS Teaching
Personnel
C. Each learner should present parents permit before the extraction and
be accompanied by their parents.
V. King Solomon Lodge No. 150 at birth shall :

A. Provide free dental services and medicines with the help and
supervision of School Head and teachers.
B. Support and guide students in assigned tasks and activities
C. The organization will provide all the necessary assistance for the
learners beneficiaries of this dental mission.

VI. CABAY NATIONAL HIGH SCHOOL shall:

A. The school will also provide Certificates of Recognition to merit the

services of the employee volunteers and the company as well.
B. Help in the facilitation of Dental Mission provided by the organization

VII. MODIFICATION

Modification within the scope of this document shall be made by mutual

Consent of the parties by issuance of a written modification, signed and
dated by both parties and approved by Cabay NHS and King Solomon
Lodge 150 at birth prior to any changes being performed.

VIII. CONFLICT OF INTEREST

Any real or perceived conflicts of interest must be disclosed to MOU.

IX. SIGNATURES

By signing below, the aforementioned parties agree to the terms of this

Memorandum of Understanding.
