SCHOOLS DIVISION OF QUEZON PROVINCE
I. PURPOSE
Pursuant to Republic Act (RA) 9155 or the “Governance of Basic Education Act of
2001” and RA 10533 or the “Enhanced Basic Education Act of 2013”, the Department of
Education (DepEd) implements programs, projects, and major activities to ensure access to
and improve the quality of basic education, an end toward which all operating units at all
governance levels have attendant roles, tasks, and responsibilities for which they are
principally accountable.
In lined with this, Lalig National High School planning team, governing council,
project teams and all of the other teams involved worked hand in hand in identifying the
priority areas of the school. Projects, activities and programs (PAPs) are crafted based on
these needs. To ensure that the created PAPs are effectively implemented and utilized,
monitoring and evaluation is further needed.
Thus, this school monitoring and evaluation plan includes projects improvements for
access, equity, quality, resiliency and well-being of the school. It further includes expected
output, activities to be done, persons involved, schedule of implementation as well as the
budget and budget source. The school M&E plan will help in informing the concerned
teams of the status of their projects. Project Teams assigned to each project are required to
submit their respective progress and accomplishment reports.

3. Gather feedback on the alignment of the Monitoring and Evaluation Tools in the
implementation of PAPs.
III. SCOPE
Lalig National High School Monitoring and Evaluation Plan along with the structures
and mechanisms, shall be implemented and spearheaded by the school planning team,
school governing council and project teams of the school. The successful implementation of
the school PAPs through systematic monitoring and evaluation will benefit the learners and
the school community as a whole.

and objectives. The post-monitoring will be schedule to be done at the end of the school
year since most of the projects, programs, and activities are still on going.
Thus, monitoring and evaluation work involves data gathering and data collection. Data
will be collected through monitoring and evaluation tools disseminated from national,
regional, division and district offices. The school will also utilize modified monitoring and
evaluation tools in its PAPs such as interviews and observation to ensure that all of the
projects, activities and programs are on track.
At the end of all activities reports for each specific project will be prepared by the
member assigned to do the task and will be consolidated into a single monitoring and
evaluation report. The consolidated reports are to be kept in the SBM corner and will be
displayed in conspicuous area of the school. The school report card of the school will be
disseminated to the stakeholders as they will also be requested to attend the state of the
school address for everyone’s awareness on the status of the school’s PAPs.
