IV. PROJECT DESCRITION
Monitoring is an activity that involves continuous and systematic checking

and observing of a program or a project. Evaluation on the other hand is judging,
appraising or determining the worth, value and quality of a program. It involves
comparing the present situation with the past in order to find out the extent to
which the laid down objectives have been achieved (Handbook for inspection of
educational institution, 2000).
As a Head Teacher and instructional director, its primary role focus is to
manage teaching staff, ensure the curriculum is followed and ensure safety and
healthy learning of students, all learners will receive an effective, efficient and timely
delivery of the curriculum. Thus, this innovation will aid in proper monitoring and
evaluation of the submitted daily lesson plan of teachers. Through monitoring and
evaluation in schools, fosters accountability and good leadership is put into practice
which leads to school improvement. Effective monitoring and evaluation can be best
achieved through record keeping and proper reporting systems, to help find out
whether the daily lesson plan is planned and executed accordingly.
For this purpose, the school shall innovate a daily lesson plan monitoring
sheet that is localized and conceptualized for all teachers of Barretto National High
School to be used by the 5 Master Teachers. The form includes subject being
taught by the teacher, curriculum level, dates in which the teacher submitted or
failure to submit the lesson plan and what are the recommendations and its
influence on lesson preparation, effective teaching and learning process and
monitoring of school attendance for teachers, effect of well planned and executed
daily lesson plan to learning environment.
Master Teachers will log the date and take note of the recommendations for
the weekly lesson Plan and this will be submitted to the office every Tuesday
attached are the hard copy of the checked DLPs of teachers. This will be done for
the whole school year of 2023- 2024.

The innovative daily lesson plan monitoring system will be pilot tested with
select master teachers, and teachers, will gather feedback and make necessary
refinements, and after which, roll out the system to all teachers and administrators
and is expected to be completed by the second quarter of the school year.
VI. PROJECT OBJECTIVES

The objective of this innovation project is to:
1. improve teacher accountability and adherence to curriculum
standards.
2. Develop an efficient and user-friendly daily lesson plan
monitoring system.
3. Streamline the process of lesson plan submission, review and
feedback
4. Enhance communication between teachers and administrators
5. Facilitate data-driven decision-making in education.

To attain sustainability of the project,
1. Active participation of the key persons in the project implementation
and development.
2. Open communication between the school head, project proponent and
master teacher during the implementation.
MONITORING AND EVALUATION
At the end of each month until the end of the second quarter, the
proponent will gather all submitted DLP monitoring sheets. Checking of the
required data in the IDOL MO Form were all filled up and summarized.
Lesson Plans submitted and logged in the IDOL MO Form were used during
one of the classroom observations. Monitoring of the use and application of
the IDOL MO Form will be done and closely recorded throughout the school
year.
Findings and conclusions will be submitted to the School Head for

further evaluation and recommendations.
