This is in connection with the PROSEC REF No. 07282021-CA-IVA-1881 (PJBC):

Patient IRENE AZUL BARLETA confined in Holy Rosary Hospital, Lopez, Quezon on 21-26 October
2017. I admitted the said patient with a diagnosis of: to consider Pneumonia/sinusitis and Myalgia
(Annex C :Face sheet). But I am not the ROD who discharged the said patient with a diagnosis of
Urinary Tract Infection/ IHD/AURI. DR. Romeo Tapales is the ROD that time that discharged the said
patient and made the final diagnosis (Annex C- Doctor Order Sheet. However I signed the Philhealth
claim (Annex A2) because it is the policy of Holy Rosary Hospital that the ROD who admit the patient
will signed the Philhealth claim.(CF2)

This is in connection with the PROSEC REF No. 04272022-CA-IVA-057 (CVB):

Patient CLARENCE LLOYD C. CANAFRANCA was confined last October 3-6 ,2014 in Holy Rosary
Hospital , Lopez ,Quezon. I am not the attending Physician of the said patient. I just made one round
for the said patient since I am the ROD that time. I am not the the ROD who admit and discharge the
said patient.

In year 2014 the policy of Holy Rosary Hospital is that all ROD that made rounds to the patient
will signed in Philhealth claim as payment for the rounds done to the patient, hence I am one of the
signatory in the Philhealth claim. (cf2

No. 07282021-CA-IVA-1880MN

Patient PRIVADO S. BORJA was admitted in Holy Rosary Hospital Lopez, Quezon ,last October 8-
11, 2017 with a chief complaint of severe epigastric pain, dyspnea with cough and dizziness. I made
an initial diagnosis of Acute gastritis /SVI based on the chief complaint of the said patient. But the
final diagnosis waS done by DR. TENORIO. However I still signed the CF2 claim because it is the policy
of Holy Rosary that the ROD who admitted the patient will signed the cf2 claim
