Nutritional Assessment and
Counseling
Preparations
▪ Familiarize yourselves with the station locations and waiting area
▪ Bring the following necessary materials:
✓Pen
✓Checklist/evaluation form (edema, MUAC, z-score)
✓WHO-CGS Table
▪ Please find your partner.
▪ A patient is assigned to each pair/group.
▪ Establish rapport with your patients.
▪ Orient the caregivers and explain the procedures that will be done.
Instructions
▪ Get the following data:
❑ Child’s name and mother’s name
❑ Birthday and computed age in months
❑ Family history (siblings, who takes care of the child, etc.)
❑ Birth history
❑Nutritional history (breastfeeding/complementary feeding practices)
❑ Health status (Is the child frequently ill?)
Instructions
▪Do hand hygiene and teach the proper procedure to the child/caregivers.
▪ Perform the following under the supervision of the facilitators:
Station 1 Measure Edema/MUAC
Station 2 Measure Weight
Station 3 Measure Length/height
Station 4 Determine the z- score/Interpret the results/Do counselling to
mothers
▪ Write the data on the form.
▪ Each pair will take turn in performing the different skills
▪ Each will evaluate each other using the evaluation/assessment checklist.
▪ Have the facilitator in every station sign the form.
▪ Sign the filled-out checklist and submit to the Station 4 facilitators
▪ Patients with medical complications should be referred to the Admin. Officer for
consultation by the physician in charge.
