Administrative Reminders:
 Registration of participants will start at 9:00am. Program begins on September 27, 2023 at 10:00 am and
ends on September 29, 2023.

 Accommodation: Triple/Quad sharing, nonsmoking room with meals.

 Room assignments are pre-set by the Department of Health Regional Office IV- CaLaBaRZon. ONLY
OFFICIAL PARTICIPANTS ARE ENTITLED TO HOTEL ACCOMMODATION AND MEALS,
OTHERWISE IT WILL BE CHARGED TO YOUR PERSONAL ACCOUNT.

 Participants should bring their original confirmation slip.

 Please confirm your attendance before September 14, 2023.

 Please inform us ahead of time if you have dietary restrictions.
