CHECKLIST OF REQUIREMENTS - HEIRS BOND

(DEPOSIT)

( ) Fully accomplished application form

( ) Deed of Extra Judicial Settlement / Self-Adjudication
( ) Fully accomplished cosigner’s information sheet (please photocopy if you need more copies)

NOTE: ALL Heirs must act as COSIGNERS and should sign the Indemnity Agreement. If NOT, or if there is only
One (1) Heir (SELF-ADJUDICATION), additional One (1) Qualified Cosigner other than the heir/s with Real Estate
property/ies and with MARITAL CONSENT of spouse, if married is required.

QUALIFIED COSIGNER:

Cosigner must own real estate property/ies worth at least double the bond amount of the bond with MARITAL
CONSENT, if married and shall submit photocopy/ies of the land titles.

Photocopies of the land titles (TCTs) of the real estate properties declared by the applicant and the cosigners in the
( ) cosigner information sheets
( ) Certificate of Publication of the Deed of Extra Judicial Settlement/Self Adjudication
Special Power of Attorney (SPA) with waiver in favor of the bond applicant signed by ALL the heirs, if it is NOT
( ) stated in the Extra Judicial Settlement
Duly registered Death Certificate of the deceased person/s whom the property/ies is/are inherited from including heirs,
( ) if necessary
Photocopy of NSO Certified Marriage Contract of the deceased person/s whom the property/ies is/are inherited from, if
( ) MARRIED
( ) Photocopy of NSO Certified Birth Certificate of children, if any
Photocopy of NSO Certified Certificate of NO Marriage (CENOMAR) of the deceased person/s whom the property/ies
( ) is/are inherited from, if SINGLE
( ) Photocopy of NSO Certified Birth Certificate of the deceased, if SINGLE
( ) Photocopy of NSO Certified Death Certificates of the deceased’s parents, if already deceased

( ) Photocopy of NSO Certified Birth Certificates of the deceased’s siblings, if NO parents are living/both deceased
( ) Photocopy of BIR Certificate Authorizing Registration (CAR)
( ) Certification issued by the bank re: total amount of deposit left by deceased
Photocopy of signature and photo-bearing government-issued IDs of ALL the HEIRS and the COSIGNERS and the
( ) Spouse, if applicable

NOTE: To avoid return of application, please submit applicable documents complete and in the same order as listed
above.
Also, please be advised that during the course of the evaluation of application, bond underwriters and/or
approvers may still request submission of additional document/requirement for further review, if necessary.
