    This is to formally authorize my sister-in-law, Ma’am Alnalyn R. Hamdain a School Head of Kepeng
    Elemetary School- Annex to receive my Cash Allowance and other Financial Assistance from your
    good office on my behalf as I am currently unavailable to do so because I have important matter to
    attend to.

    Attached herewith is my photocopy of my government issued ID with my signature to claim my said


    Financial Assistance for your easy reference. This authorization letter shall ONLY be valid once to
    claim it for the date mentioned above.

    She may also sign any documents pertaining to the released of said financial assistance in my behalf.
