Ayan is a 13 years old girl who has a sprained (twisted) ankle. As you are
examining her she asks if you can prescribe the contraceptive pills for her.

On examination she has swollen ankle but nil else of note. She looks slightly older
than 13 years.

TASK:

Carry out a consultation with Ayan about her request

Actor instructions:

You are a 13 years old girl living with her family your parents are divorced with

father having left the family 5 years ago. You live with you mother and mother’s

partner for 2 years.

You also have a brother aged 11 years and sister aged 5 years. Your boyfriend is

friend of your mother’s partner. You don’t want your mother to know about this

relationship. As you feels your mother will be jealous. Your boyfriend has asked

you to keep the relationship as a secret so that it will be more special.

You are a doctor in the out-patient clinic and you see a 27 year old man who has

epilepsy.

He has a history of complex partial seizures he has simple auditory hallucinations

during aura. Prior to being on antipsychotic medications, after the seizures he

would typically have a day when he felt well, and then would become unwell with

auditory hallucinations persecutory delusions, low mood and irritability. This

would last for a few days. In the past he has also reported seeing fleeting images

of people laughing at him. 3 years ago haloperidol was added to his antiepileptic

medication phenytoin. His medication has kept him reasonably well but he is

troubled with extra pyramidal side effects and has come to see you about him.

TASK:

Write down clearly what you want to know in order to make decisions about his

medication?

A 75 years old woman had been diagnosed with dementia 10 years ago. She was a
banker.

Her memory is getting worse in the last month. Her daughter asks you to assess
her.

Actor instructions:

You are a 75 years old woman from Borama with a known history of dementia for
10 years. You speak fluent English.

You don’t know where you are , who you are , you remember your name 4
minutes later. You don’t know time. You don’t know when asked where you are
in. you don’t know this day, this month, this year , this season, the town you are
in, the floor you are in , and the country you are in. You are asked to remember
for 3 things. You remember 1 out of 3 . You can’t repeat the others.

You are asked to subtract 7 from 100 for 5 times. You don’t know any of these.
You are asked to read the word world from backwards. You can’t do that.

You can’t recall 3 objects asked previously. You name 2 objects that you are asked
to name.

The candidate asks to repeat No ifs and buts and you say after it. Candidate shows
you a paper saying close your eyes but youdon’t follow this command. You are
offered to write down a sentence but you cant do so .

You are asked to copy a picture but you can’t do that.

You are a 6th year student doing a placement at the GP. Jessica is a 35-year-old female
who has come in with concerns about her excessive alcohol consumption.

Task

In a total of eight (8) minutes:
