Abstract
.Our clinic is limited on homemade workshop. This has limitation on controlling the work securely, for
declaring the result on time, and has high consumption on coffers. Homemade data processing
system isn't fast and effective way to negotiate tasks within short period of time and it's boring. To
overcome this problem we proposed the new automated system. The proposed system includes
Patient Information, Pharmacy, Laboratory and Clinic Personnel Information. We envisaged an Bule
hora university Clinic that isn't as similar well- organized clinic, However it stands for serving
patient scholars and security of their life as well as for the purpose of precluding different
conditions. It works anyhow of patient information recording, making treatments over different
conditions and it gives referral for cases to another sanitarium for fresh treatments. In order to do
this design first we've collected data relating to being primer grounded system using data collection
system like canvassing and document analysis. After that we designed the demand we've collected
by using rapid-fire operation development( RAD) model and we also used some assaying and
modeling fashion edrawmax and Microsoft Visio for designing. Eventually the overall exertion of this
phase of the design work is about design and modeling of automated Bule hora clinic case’s
information operation system

. CHAPTER ONE
INTRODUCTION

1.1 BACK GROUND OF THE STUDY


In today world, there's a need for automating the system of data keeping, so a lesser need for an
automated Clinic case’s Information Management System. The information technology revolution
coupled with everyday use of computers in clinical has created new demand for electronic case records.
The benefits, particularly for large clinical institutions, are egregious bettered record control, easier
document storehouse and access, and better information for clinic operation. Eventually, the main
purpose of automating Clinic case’s Information Management System is to ameliorate health care
quality.

Technology is spreading its sect in nearly every walks of mortal life conditioning. Now a day, it's better if
every exertion is done using new technology in order to fulfill the need of mortal being, Organization and
Enterprise etc. As moment’s world there are numerous associations and each associations needs to be
preferable, computable and work on fastest way in order to satisfy druggies interestetc. i.e. they should
have grease their conditioning in motorized way.

in automated Clinic case’s Information Management System is computer software product that
coordinates and integrates all the essential conditioning involved in the operation and handling of a
healthcare installation. They must meet specified security, technology and functionality norms for
managing electronic medical records and practice operation information. It's created to computerize
homemade operations in clinic. The primary purpose is to digitize patient records so as to make data
reclamation easy and effective. Being in the digital form, patient data can be accessibly participated and
penetrated by multiple contemporaneous druggies at different locales, performing in clinical operations
and collaboration among clinicians.

Economically, clinic benefits from constant cost savings as a result of increased productivity and overall
effectiveness. Basically, everyone in the clinic benefits from the system- croaker , nanny , clerk, director
and the clinic proprietor which makes their lives easy and removes gratuitous mortal crimes from their
diurnal conditioning.

1.2 MOTIVATION OF STUDY

presently, there are a lot of conventions in our country. People who suffer from sick orpain will visit
the clinic which hard their position. still, ultimate of the staff in theclinic still using paper factory in
their workflows. Because of all of these manualoperations are done by numerous babysitters in a
clinic, it may brake down the procedure in manyway. For illustration, when the croaker uses manila

a lot of factory are suitable to automation and noneed manual operation. Clinic operation system is
one of the software that available inthe request and suitable to help the business process of a clinic.

so we want to solve those and others problem by developing Automated clinic patient information
managment system.

1.3 Statement of the Problem

As per face-to-face interview with the clinic personnel, personal observation, experience and
initial analysis, the existing Clinic Office has the following drawbacks:

⦁ There is a work load to the clinic personnel

⦁ Data records are not secure;

⦁ Searching for even single data is time consuming.

⦁ Prone to loss / misplace of patient record.


⦁ Patient is not getting services timely according to their arrival and appointment.

⦁ Patient records are not well organized due to its volume, capacity of the filing space and
the number of office clerks.

⦁ Inadequate records in the pharmacy section inventory system.

⦁ Redundancy of patient data.

Currently, croaker and nanny worry about the available system is because it's notquite popular
and it needed some specific chops in order to install or operate it. the system that we are going
to developing will resplace the manual work of paper. The current clinic operation system that
available in themarket is just replace the paperwork by using computer and store the data into
thedatabase. The normal functionalities include the case record operation module, force
module and billing module. There isn't important new functionality that availablein the system.
In this design, I intend to break the problem that the croaker and nurseworry about in the
available system and add in new inventions that help the case andstaff of clinic in my own
advanced system

1.4 objectives of the proposed system.


1.4 .1 General Objective.

The main ideal of proposed system is to develop an automated Clinic Case’s Information Management
System for our country which solves the over mentioned problems with the being system.

The main objects of clinic operation system is to ameliorate the currentavailable system that warrant
some inventions which are more likely needed in the clinic.

To give a methodical schedulesystem.

1.5 Significance of the research..


The proposed system will aids the clinic to negotiate their tasks similar as patient enrollment ,
storing and reacquiring patient information as well as generating an systematized report in
effective and effective manner. To insure the system is secure enough to help any unauthorized
person access to the case’s record. Only authorized stoner has the right to recoup data of their
own. To eliminates the need of jotting and rewriting the patient information. To reduce
workloads of Clinic Personnel.

.At the end of this design, the new functionality in each module will be suitable tosolve numerous of the
problems. By having these modules in the system, case that suffer insick no need to waste a lot of time in
the clinic to see a croaker . This system is suitable todecrease the time of a case staying in the clinic and
also reduce the workloads for anurse and croaker when serving thepatients.In addition, with the
appointment module that integrated SMS announcement, it alsohelps to help the case for not being miss
the appointment with croaker . Through thisfeature, cases no need to worry about they missed out for
the appointment and need torearrange appointment again.

1.6 scope of study.


The scope of the project encompasses the design and development .It has four modules listed
below;
· 1.Patient Information module

· 2. Laboratory Information module


· 3. Pharmacy Inventory module and

· 4. User Management module

· This design aims to develop a clinic operation system for the clinic thatlocated in
Malaysia which is still using the homemade operation.

· This system can be used by croaker and nanny . It suits for any periods of cases
especially the oldfolks.The clinic operation system includes a combination of
modules and utmost of the basicmodules formerly done in the available system in the
request.

. Case is suitable to make appointment through the phone and thestaff will record it in the
system. After that, system will use the SMS point to notify thepatient on the day
ofappointment.Registration module.

· When a case wants to see a croaker , first he or she needs to inform thecounter and
the nanny will add the case into line list. The line list module willcomes with a time
tracking for each case to know how long they're staying in theclinic.

CHAPTER TWO

LITERATURE REVIEW
2.1 Introduction of the Existing System

In this chapter we can determine how the current system functions and assess what are the
functional and non-functional requirements for the new system. The purpose of analyzing the
existing system is that to determine exactly how the existing system works, to identify what
problems exists, and finally to give a solution. The aim here is to describe the existing system,
and what the actors of the existing system are. And also describe the business rule of the
proposed system,
2.1.1 The actors (players) of existing system

The most common actors are:

· Manager: is the one who manages and controls all activities in the clini

· Patients: patient goes to the clinic and do the following activities

· Get registered

· Tell about his disease to nurse

· Depending on his symptom the patient is ordered by the nurse.

· Nurse: The nurse gets reports from the pharmacist, and Lab technician. The reports help
the manager to see how services are given to the patients to view patient information.

· Pharmacist: The patients come with the ordered prescription then the pharmacist looks
that order of drug and gives the drug accordingly. The patients get his/her requested
service from the pharmacist. Pharmacist is responsible to register, modify, remove
outdated the drugs in the stock.

· Clerk: To register the new patient and the previously registered patient come for further
treatment, they will tell card number and the clerk or receptionist search for that specific
card from the stored catalog of the cards

.Lab Technician: To conduct laboratory testing and generate the lab report.

2.1.2 Functions of the Existing System

· most of our clinics is using the paper based system as a means in recording patient and
workers information.
· When a new patient visits the clinic to avail treatment for registering, as well as searching
of existing patient record is also paper based.
· The patient arrives at the clinic, the patient will provide information (Name, ID, Age, and
Dept.) and the patient will be given a medical card.
· There is also another medical card (chart) that contains the patient information and the
report of the treatment.
· The patient medical document is stored in the catalog in card room based on the medical
card number assigned to the patient.
· When the previously registered patient come for further treatment, they will tell card
number and the clerk or receptionist search for that specific card from the stored catalog
of the cards.
· The clerk gives to the doctor or nurse the form card after registration and directs the
patient to the outpatient department for consultation and diagnosis and then the nurse
order the patient to another departments either to laboratory department for examine the
disease or to the pharmacy department to take necessary drugs.

2.1.3 The problems of being system


2.1.3.1 Performance
⦁ Response Time
⦁ The response time to do a given task in the current primer grounded system is significantly high
because of the time it takes to get the applicable information of case manually is time taking.
⦁ That means the systems are searching and getting available patient information are requested for
use takes long time.
⦁ The case doesn't get the service timely.
⦁ As we estimated the time taken to serve each case is at least one hour per a day.

2.1.3.2 Information
⦁ Out Put It's delicate to produce applicable patient information using the being primer grounded
system
⦁ Input occasionally the data isn't directly recorded
⦁ Stored Data Case record information isn't secure Unauthorized person can pierce patient record

2.1.3.3 Effectiveness.
⦁ The being primer grounded system isn't important effective and not optimal because it consumes
further coffers to perform tasks and the data may be exactly recorded.

⦁ The system isn't well organized to perform each conditioning and it isn't flexible to change or
modify the data whenever demanded.

METHODOLOGY OF STUDY

3.1 Data Source:

⦁ We get further information from bule hora university clinic director and pupil.
⦁ Practical observation enables us to know and list out the being system problem
⦁ We bandied real problem of the being system with our platoon member

. 3.1.1 Fact Chancing ways

Interview- This is one of data collection system that enables to gather information from the
association directly in the form of asking question to clinic labor force similar as the nanny ,
croakers , druggist and others, we elect the interview system because the clinic is set up in
our lot, so that it's better to go and ask an information that's essential for our design.
Document analysis- we also collected certain applicable information from written
documents in the clinic.

3.1.2 Methodology for the system design and development:

In any software development exploration, a methodical methodology, or process, should be


followed through the life cycle of the exploration to insure thickness and absoluteness.
For this exploration, we used Rapid operation development( RAD) for the System
Development Life Cycle( SDLC) as seen in Figure 1. Rapid Application Development has
four essential aspects methodology, people, operation, and tools.( 1) If any one of these
constituents is shy, development won't be high speed. Development lifecycles, which weave
these constituents together as effectively as possible, are of the utmost significance.( 2)
⦁ Methodology
The RAD development path attacks these challenges head on by furnishing a means for
developing systems briskly, while reducing cost and adding quality.
⦁ People:
The success of Rapid Application Development is contingent upon the involvement of
people with the right chops and bents.
The crucial players in a Rapid Application Development design include:
Finance: A high- position stoner superintendent who funds the system and is devoted to
both the value of the new system and to achieving results snappily.

Conditions Planning Also known as the Concept Definition Stage, this stage define the business

3.2 system analysis


System analyzing and modeling is the essential part of the system development. To develop
online clinic patient’s information management system analyzing and modeling is needed. The
system has four modules (user management system, patient information system, pharmacy
information system and laboratory information system)and every actors in each modules have
their own function (use cases).by having these module of system and the concept of modeling in
the mind we have modeled the system using use case ,object ,dynamic model and user interface
model.

The following are the identified actors (users) that will be participating in the system with their
respective activities.
· System administrator: is the one who manages and maintains the system.
Main task of system admin/manager includes:

· Manages user accounts, register user, search, view, update, and delete user information
· Nurse: A person skilled or specializing in healing arts.
Main task includes:
· Treat patient and access patient information
· View reports, update patient information and write prescription letter
· Pharmacist: -Manages the drug information in the store.

3.2.1 existing system


The clinic backing must treat cases in good manner

⦁ director should control the entire exertion in the clinic and should admit clear and applicable
report from the workers of the drugstore and laboratory.

⦁ Forms should contain cases and stock information meetly.

⦁ druggist doesn't give departed medicines to the case

⦁ One case should have one and only one patient number that's the same with the given university
pupil id number.

As needed, patient are advised by the doctor or nurse to undergo laboratory testing. The
Laboratory Requesting Form below in Table2 is used to endorse patients to undergo the said
laboratory exams.

2.1.4.4 Clinic Prescription Forms used in the Existing system

The Clinic Prescription Form in Table 3 is written by the physician / doctor or nurse directly
addressed to the Pharmacy Section. The form indicates the prescribed medicine or drugs to be
administered to the patient.

As needed, patient are advised by the doctor or nurse to undergo laboratory testing. The

addressed to the Pharmacy Section. The form indicates the prescribed medicine or drugs to be
administered to the patient.

3.2.2 Proposed system
The purpose of the new proposed system is to break the problems in current system using a web
grounded information operation system and to determine the conditions demanded to design and
develop a Clinic case’s Information Management System.
By precisely assaying and observing the problem of being system we came up with a result all the
downsides of the being system and also enhance the way of operation to make the operation
performing easy. The proposed system will exclude/ reduce the problem on time, work cargo and
complexity on storing patient information. The time for reacquiring the information demanded will
be less compare to the primer. This will help the clinic adjunct in doing the job. The system will
include a database for recording case that grease fast information reclamation, modifying, fitting
and deleting. It also includes an seductive stoner interface that facilitates penetrating the database
and recording case fluently. The proposed system also includes the druggist where anyone can
interrogate about the medicines vacuity and the stock to be ordered as well as about its expiry date.

When doing these research we hesitate the following important points:


⦁ Reduce the time and task needed to perform the operation within the clinic office.
⦁ It'll change the primer processing to computerize system.
⦁ It'll give speed, effective, Inflexibility, trustability, and security for the system druggies.
⦁ For clinic labor force, better satisfaction of the speed handed by the system.
⦁ And it bettered the moral( provocation) of the druggies to use the new technology.
⦁ Performance The performance of the proposed system provides fast response time because it's
easy to pierce data from the stored document.
⦁ effectiveness the web grounded system by itself is short and clear and in this system there's no
duplication of data through the new system so it's important to manage effects around patient
information.

3.3 Feasibility Analysis

This system bring better achievement for the operations performed by the clinic by furnishing
effective enrollment and storehouse of patient information, easy updating, omission, revision etc.
And it also increases the effectiveness of work in the clinic. So that bone can say that the system
is operationally doable.

3.3.2 technical analysis


The proponents of this study are confident and able, with respects to their specialized chops and
the vacuity of the tools to materialize this design. The system to be developed by using
technologically system development ways similar as PHP, Java script, css and MySQL database
without any problems and the group members have good capability to develop the design. So the
system will be technically doable.

3.3.3 economic analysis


It's delicate to develop the system in our budget. Because the proposed system wants veritably
high price.
The system to be developed is economically doable and the benefit is overbalancing the cost.
Since this exploration formerly computerizes the being system, by now the reduction of cost for
accoutrements used in homemade operation becomes devisee to the association.
Generally the system that we developed, clinic case’s Information Management System brought
a number of palpable and impalpable benefits.

3.4 System demand of the proposed System


3.4.1 Functional Conditions

Functional demand is the services that are handed by the system. It also describes the
relations between the system and the stoner, and any other external system. The new
system is anticipated to give the following functionalities.
⦁ Register new case: our proposed system can register each case and records their
introductory information one database. This helps the clinic to manage and manipulates
cases ’ information fluently and to store their data and documents permanently in database.
⦁ Hunt for being case from database: This enables the druggies of the system to find patient
information from database which is preliminarily registered. When formerly registered
case come for farther treatment, the responsible person should search for that specific
patient information.
⦁ Modify patient information: The authorized druggies of the system can add, cancel,
modify or change case’s information whenever possible.
⦁ Viewing and penetrating patient information: The system can display or recoup the stored
introductory information of the case’s record information, reports and records which
stored in the clinic database. An authorized individualities can view and pierce some or all
of the cases ’ medical treatment information more fluently.
⦁ Make patient appointment: The clerk of the clinic using the system can make appointment
for a particular case grounded on the vacuity of the croakers . They can fluently set the
appointment date and time.

Generally our clinic is limited on homemade workshop. This has limitation on controlling the work
securely, for declaring the result on time, and has high consumption on coffers. Homemade data
processing system isn't fast and effective way to negotiate tasks within short period of time and it's
boring. To overcome this problem we proposed the new automated system. The proposed system is
ambo university institute of technology clinic case’s information operation system and it includes
Case, drugstore, Laboratory and Clinic Personnel Information. The clinic has three nursers, two
laboratory technicians, two druggists, and two clerks.
We envisaged an our Clinic that it wasn't as similar well- organized clinic; still it stands for serving
patient scholars and security of their life as well as for the purpose of precluding different
conditions. It works anyhow of patient information recording, making treatments over different
conditions and it gives referral for cases to another sanitarium for fresh treatments.
The design is partitioned into five chapters and each chapter has a specific deliverable which is
essential and base for the coming chapter. But the platoon have tried its stylish and developed
intriguing system web grounded IoT clinic case’s information operation system for Ambo
University. It's flexible, accurate and seductive with easy GUI approach. The system performs works
better than being primer grounded system( usability, speed, effectiveness and effectiveness).
Security also included in the system developed and only authorized stoner can pierce the needed

want to recommend that this system has to be changed to Android grounded system as druggies use
