 Heart rate: 78b/m
 Weight: 50lbs
 Height: 5’5”
 Unremarkable physical exam
Context of psychosocial issues:
 Single parent with 2 children
 Considers health as last priority
 In need to earn support for needs of kids
Analysis  Patient who is presenting with clinical
symptoms of Diabetes Mellitus with an
FBS of 150mg/dl, and am HbA1C of 7%
 Poor health prioritization

Management/ Intervention  Determine serum creatinine, BUN, AST/
ALT, uric acid, lipid profile
 Do 12-lead ECG
 Metformin therapy: 5oomg PO BID
 Periodic measurement of vitamin B12
levels
 DM diet
 Individual counselling (Catharsis-
Education-Action) to address the
identified emotionally critical
misperceptions
 Elicit individual preferences and beliefs
 Provide emotional support and available
support programs for single parents
