JOINT AFFIDAVIT OF NON-FILING

We, ________________ and ______________________, both of legal age, married,

Filipino Citizens, and both residents of Catabayungan, Cabagan, Isabela, after having been duly
sworn to in accordance with the law, hereby depose and say:

1. That we are the parents of ________________________;

2. That ________________________ is currently enrolled as Fourth Year College student in

CSU Caritan, Tuguegarao City, Cagayan;

3. That our said son is applying in KUYA WIN SCHOLARSHIP PROGRAM;

4. That one of the requirements is a proof of the Bureau of Internal Revenue (BIR) validated
Income Tax Return (ITR) of the parents;

5. That we did not file our ITR at the BIR because we are not employed, and only receiving
pension from Social Security System (SSS);

6. That we are executing this affidavit to attest to the truth of all the above facts and
statements for all legal intents and purposes it may serve.
