The AcciMap analysis process involves a systematic approach to identify and understand the
causes and contributing factors leading to incidents and accidents. The process can be
summarized as follows:
Data Collection: Gather information related to the incident or accident through interviews,
report analysis, and observation recordings.
Identifying Failures: Identify failures at all six levels of the AcciMap structure:
Identifying Factors: Determine the specific factors related to the incident at each of the six
AcciMap levels.
Determining Causal Factors: Analyze the identified factors to establish causal relationships and
consequences. Create a preliminary diagram.
Constructing the AcciMap Diagram: Use the data and causal relationships to create the
AcciMap diagram, visually representing cause-and-effect chains.
Review and Refinement: Review the constructed AcciMap diagram, making adjustments and
refinements as needed through iterative iterations.
When applying the AcciMap approach to severe outcomes or near-misses, follow these
additional steps:
Determining Contributing Factors: Identify the factors contributing to the adverse outcome
based on incident reports and data.
Placing Factors in AcciMap Levels: Assign each contributing factor to the appropriate level in
the AcciMap structure.
Filling Missing Gaps: Address any missing information in the causal chain to ensure a
comprehensive analysis.
Assessing Causal Logic: Verify the logical sequence of events leading to the adverse outcome.
