:Additional Guidelines for completing the application form
Please add all the supporting documents that must be attached, according to the nature of the request, for example:
a. Official documents and public certificates attesting to the relationship between the foreign citizen and the inviting party (birth certificate,
marriage certificate, etc.).
b. Photocopy of the foreign citizen's passport (which must be valid for at least 6 months) including entry visas to other countries visited by
the guest.
c. In the case of entering due to medical circumstances - when submitting the application, up-to-date medical documents must be attached.
Firstly, they must be written a month prior to the application; and secondly, they must be signed by a public hospital, clinic of a public body or
other official body detailing the applicant's current medical condition.

 I declare that the particulars contained in this application are correct and have been made in awareness of the fact that they are
to serve as basis for the consideration of my application. I also declare that I have not committed any criminal offence or any act
directed against the security of the State of Israel and that I am not affected with any illness which might endanger the public
health. There is no judicial warrant of arrest against me and I am not wanted by the police of any country.

 I undertake that my stay in the West Bank will be in strict compliance to the terms of my visa and agree to ensure to leave the
West Bank upon the expiration date of my visa/license.

 I know that upon the arrival of a minor (foreign or resident) with one of his parents or any other guardian, they are required to
present an original birth certificate of the minor, as well as a photocopy of a passport / ID card and an appendix of both parents.

 In cases where the person inviting a foreigner whom has submitted similar applications in the past, it must be ensured that his
guests have left the area and met the visa conditions. If the guests did not leave in time or at all, the request will automatically be
denied.
