Dear Mr Zanetta,
Re: Notification that you may not be eligible for Diabetic Eye Screening.
We are writing to inform you that we have received notification from you or your GP surgery
informing us that you are not diabetic and therefore are not required to attend for Diabetic Eye
Screening.
For this reason we have written to your GP surgery requesting they review your diabetic coding
along with completing a Not Diabetic Form. If this information is correct, you need not do anything
further.
If you feel that this is incorrect and you are diabetic or have been diabetic, please contact your GP
as soon as possible to review your medical records. The GP surgery will need to update the
diabetes coding on your record. If we do not recieve confirmation from your surgery we will write to
you with another invitation.
