AFFIDAVIT

I, ROSEMARIE J. AQUINO, of legal age, married and a resident of 123 Holy Family
Avenue Holy Family Village, Bagumbayan, Taguig city. hereby state under oath that:

1. I am the Barangay Secretary in Barangay Bagumbayan, with office address at Purok 3 MLQ
Street, Bagumbayan, Taguig City. My duties is to abide the other errands of the Punong
Barangay Carmi Ogalinola-Pajarin include the remaining scrap materials turned over to our
barangay.

2. By virtue of subpoena dated September 28, 2023 issued by the Office of the Ombudsman
hereto attached as Annex A, I visited the location of the scrap materials on April 26, 2022 to
30, 2022 to gather documentary evidence and to conduct interviews, information elicitation,
and ocular inspection relative to CBU-C-JUN-23-0026.

3. During the inspection, I took pictures of the scrap materials and documents showing its
current status using my cellphone _____________________ (cellphone brand) model with
Serial No. __________. The photographs were captured in a Secured Digital (SD) Card,
with model: _________, capacity: _____GB, and serial no. ________________, with the
following tags:

4. I stored the raw files of said photographs under the folder file name: ____________ in the
hard drive of my computer laptop, with model _______________, Serial No. ___________
and Part No. ___________ for custody, record keeping, evidence, and printing purposes, all
in connection with CBU-__________.

5. After the inspection, I printed the photographs using [details of the printer used]. The print-out
of the photographs are hereto attached as Annex/es [xx].

6. The attached photographs have not been altered or modified in any way, and are true and
faithful representation/s of the pictures I took on _________ 2023.

I am executing this affidavit to attest to the truth of the foregoing.
