First Inspection and Audit Defect
Categorisation Manual
For a full explanation of defect coding refer to Engineering SOP 2102.
Note: A component should only be considered defective if it has clearly reached the
stage where repair or replacement is required either immediately or will reach that
stage prior to the next inspection.
“X” Category Defect must be reserved for “Dangerous deficiencies” constituting a direct and immediate
risk to road safety or having an impact on the environment, that would attract a prohibition from the
relevant traffic authority or would be a reason for rejection at MOT.
“D” Category Defect must be reserved for “Major deficiencies” that may prejudice the safety of the
vehicle, have an impact on the environment, put other road users at risk or other more significant non-
compliances. Or:
• Smoke levels greater than VOSA’s smoke test threshold
• Defect(s) that would lead to an imminent breakdown
• Any defect on First Bus Safety Systems
Any D Category fault that does not require immediate repair may be deferred and planned for a later
date, prior to the next Vehicle Safety Inspection. In all cases where this occurs an auditable trail must be
present using First Bus engineering documentation.
“A” Category Defect. Minor – deficiencies having no significant effect on the safety of the vehicle or
impact on the environment and other minor non-compliances. If only defects of a minor nature are
present, a test certificate will still be issued.

The way forward is simple “ If you cannot do it safely, don’t do it”

Note 2: All recorded brake test results must be fully examined, and ‘PASS’ must not be relied upon. For
completeness each independent brake reading must be assessed to ensure that the vehicle is fully roadworthy.
Note 3: Imbalance failure will not apply if both wheels lock, or if one wheel locks and the brake effort of the locked
wheel is less than the unlocked wheel

Secondary Braking Performance VOSA IM Ref. 72

The brake force requirements for each individual vehicle can be normally found on the PSV technical prints, from the
Computer technical records or from T.S.S. Branch at Swansea. If this information is not available the adequacy of the
braking performance is established in one of three ways depending on the number of wheels which lock during the roller
brake test, as follows:

Calculated Gross Vehicle Weight (CGVW) is found by using the formula CGVW = X + 63.5 (Y+Z)
where: X is the unladen weight.
Y is the number of passengers that the bus is constructed to carry (in addition to the driver).
Z is for a bus with standing capacity more than 8, the number of standing passengers minus 8.
All weights are in kilograms.

Note: If no wheels lock use Gross Vehicle Weight column to obtain pass figures.
If one of two of the four wheels lock use Unladen Weight column to obtain pass figures.

X - c) Imbalance across the axle exceeding 30%
Note: For performance figures see the previous tables. All
figures are calculated to 30% of the relevant vehicle weight.
Note: There may be occasions where the specified brake
effort is met but, if the brake effort from any wheel on an axle
is substantially less than the brake effort from another wheel
on the same axle, it may be necessary to consider reason for
failure

Brake Performance Calculations

Brake Performance Criteria

Brake performance is calculated by the total braking force at the rollers in kg, divided by the weight of the vehicle,
multiplied by 100. eg: Total braking force = 9,500kg, vehicle weight = 15,654kg

9,500 x 100 = 50% performance

15,654

1. If no wheels lock during a brake roller test use the calculated gross weight or design gross weight of the vehicle to
calculate the brake performance *(see note at the bottom of the page).
2. If one or more wheels lock during a brake roller test use the unladen weight (stated on the N/S of the vehicle) to calculate
the braking performance.
3. If three or four wheels lock, the brake performance is considered to be met.

* Vehicles Post 1982

Use UK gross vehicle weight (GVW) this is normally stated on the vehicle chassis plate.

Note: Pass figure can be found in the following appendix.

Pass figures are calculated to 60% of the relevant vehicle weight.
