due to MEMBER/PATIENT ALREADY DECEASED AS PER RECORD, with details as
follow:

Name of Patient: Miranda, Caridad

Date of Admission: March 26, 2021
Date of Discharge: March 26, 2021
Amount of Philhealth Claim: Php 2,600.00

Please reconsider this claim. The patient was still alive during this confinement
period. Her last session of Hemodialysis as of this writing was on August 3, 2023.
Someone might have erroneously tagged her as an expired patient during one
of her dialysis sessions, the reason for her appearing in your system as deceased.

We attached all the relevant documents for your further evaluation, including the
Certification from our Dialysis Unit.
