1. Assemble equipment.
2. Introduce yourself. Explain what you are going to do, why it is
necessary and how the client can cooperate.
3. Perform hand hygiene.
4. Provide for client privacy by drawing curtains or closing the door.
ASSESSING TEMPERATURE
5. Place the client in the appropriate position.
6. Place the thermometer depending on the type of thermometer being
used.
7. Wait for the appropriate amount of time.
8. Read the thermometer and document the temperature.
ASSESSING RADIAL PULSE
9. If client is in supine position, place client’s forearm straight alongside
or cross lower chest or upper abdomen with wrist extended straight.
If sitting, bend client’s elbow 90 degrees and support lower arm on
chair. Slightly flex the wrist with palm down.
10. Place tips of first two fingers of hand over groove along radial or
thumb side of client’s inner wrist.
11. Determine strength of pulse. (bounding, strong, weak or thread)
12. Count the pulse for 60 sec. or 1 full minute.
ASSESSING RESPIRATION
13. If you anticipate client’s awareness of respiratory assessment, place
client’s arm across the chest and observe the chest movements while
supposedly taking the radial pulse.
14. Count the respiratory rate for 60 sec. or 1 full minute.
15. Observe the depth, rhythm and character of respiration.
16. Document.
