during the mechanical or tes ting in HB 2nd Mezza nine, Clinical Lab Area

Dear Sir,

Subcon tractor acknowledges the Contractor's Work Request Form dated 23 November 20 14 with
th e instruction to th e Subcontractor to arrange for the allendance of one personnel to coordinate
on site during th e mechanical OP testing in HB 2 0d Mezzanine, Clinical Lab Area.

Subcontractor wilt refer to this particular leller as Work Request Form No. 18 ("WRF No. 18") for
ease of reference/identifica tion and reiterates its request to th e Contractor to assign reference
numbers in future to beller manage such Work Request Forms.

Subcontractor notifies th e Con tractor that th e above instruction invo lves additional work outside the
Subcontractor's original scope of wo rk which en titl es th e Subcontractor for addi tional time and
cost.

Subcon tractor requests th e Contractor to advise wheth er a new change requ est wilt be issued fo r
th e above.

The above is necessary to understand the method for claiming payment for th e works in the
m eantime Subcontractor notifies th e Contractor th at it progressed with the work .

Please find the attached allendance record of our personnel duly signed by the Contractor's
representative with the cost estimate which amou nts to QAR 642 .20 (Six Hundred Forty Two and
Twenty Dirhams) for Contractor's assessm ent.

Dear Sir,

Subcontractor acknowledges the Contractor's Work request Form dated 26 November 2014 with
th e instruction to th e Subcontractor to arrange for th e attendance of one personnel to coordinate
on site during the mechanical DOP testing on 26 November 2014.

Subcontractor will refer to this particular letter as Work Request Form No. 21 ("WRF No. 21") for
ease of reference/identification and reitera tes its request to the Contractor to assign refe rence
numbers in future to better manage such Work Request Forms.

Sllbcontractor notifies tile Contractor that th e above instruction involves additional works outside
the Subcontractor' s original scope of work which entitles the Subcontractor for additional time and
cos t.

Subcontractor requ ests th e Contractor to advise whe th er a new change req uest will be issued for
those works.

The above is necessary to understand th e method for claiming payment for the works: in th e
meantime Subcontractor notifies th e Con tractor th at it progressed with th e works.

Please find the attached attendance record of our personnel duly acknowledged by the
Contractor's representati ve and the cost estimate Wllich amounts to QAR 64 2.20 (Six Hundred
Forty Two Riyal s and Twenty Dirh ams) for Contractor's assessment.

Subject: Work Request Form No. 35 - In struction to silence and reset the Fire Alarm as per
SIBS requirements at Staircase 4. OPC Building

Dear Sir,

Subcon tractor acknowledges th e Contractor's Work Request Form dated 16 December 201 4 which
was received on th e 17" of December 201 4 with th e instruction to silence and rese t the Fire Alarm
as per SIBS req uireme nts for th e tes ting and commissioning of Staircase Pressurization fan, OPC
Building.

This parti cular Work Request Form will be referred as Work Request Form No. 35 ("WRF No, 35")
for ease of reference/identification and req uests Contractor to assign reference numbers in future
to better manage such Work Request Forms.

Subcontractor notifies the Contractor that th e instruction in WRF No. 35 involves additional works
ou tside the Subcontractor's original scope of work and not included in any previous instructions
which entitles th e Subcont ractor for additio nal time and cost.

Subcontractor requests the Contractor to advice whether a new change request will be issued for
th e additional works,

The above is necessary to understand the method for claiming payment for th e works: in th e
meantime Subcontractor is progressing wit h the works,

Please find th e attached cost estim ate for th e subject WRF No. 35 whicll amounts to QAR
2,408.25 (Two Thousand Four Hundred Eight Riyals and Twenty Five Dirhams) for Contractor's
assessrnent.
