Subject: Issue of temporary entry gate passes.

Dear Sir,

We have been a warded the above contract no. LOA no. 9030010139 ANK/ ANK-ASSET/ SUPPORT/ MM/
PURCHASE/ 2020/ 612355/ A15XC20001, Dtd. 05.03.2021.

For the performance of which we required to enter into the premises of the GGS GNAQ of ONGC Ankleshwar,
for the perform of the said contract. We would therefore request you to please enclosed Duty/Entry passes to
be issued to our employees to enter into said/Unit/installation/office.

The names and other details of these employees are annexed, the passport size photograph of each of our
employees for whom the duty/‘entry passes in sought are also enclosed for your record. We as employer of
persons for whom duty/Entry passes are sought assume full responsibility.

We do by undertake that,
1- Endorsement of Duty/entry passes shall not impose any liability of ONGC
2- We have scrutinized the previous working of the persons proposed to be employed and there nothing
adverse as regards his/her character and antecedent.
3- The character and antecedent of the person proposed to be deployed is/are impeccable

UNDERTAKING

M/s ESP Safety Pvt. Ltd. giving this undertaking before deploying of manpower at project site ONGC Ankleshwar.

1. We will get medical fitness test certificate from medical officer (MBBS doctor) of all the operators before
deployment at project site in view of current situation of COVID-19.
2. Our team will follow all the safety terms including use of all PPEs to avoid current situation of COVID-19.
3. All contractor workers coming for the work will be staying near worksite for all period to be decided by the site
in-charge . We shall be maintaining a separate log register feels crew.
4. Use of Arogya Sestu application will be used by our team and encourage the family members for the same.

5. Wearing face mask shall be compulsory at sire for our team. We will provide face mask to our team and will
ensure not enter without mask inside premises.

6. Anybody found with temperature at more than 99 F, Fever, cough breathing problems will not be allowed to
enter the premises and will be taken to doctor.

7. On doctor advice if necessary the operator will be send to designate isolation center and all arrangement will be
done by M/s ESP Safety Pvt.Ltd.

8. We will make sure to keep working area clean & hygienic.

9. All operators will be advised to clean and wash hand with soap before and after ever meal and at regular
intervals.

10. We will take almost care to inform management and local administration if any operator found COVID-19
positive.
