INTRODUCTION

The Client PT. PERTAMINA GEOTHERMAL ENERGY intends to develop the Lumut Balai
geothermal field in South Sumatera with the second 55 MW Unit, also referred to as Power Plant
Unit 2 (U2) with its related Fluid Collection and Reinjection System (FCRS), and to extend the
existing Reinjection Line of Unit 1.

Consortium MC&MHI, SEPCOIII& WIKA is awarded with the EPC contract by Client as Contractor.
The project is based on a form of contract (EPC) in which the Consortium is to design, procure,
construct, commission, complete the Plant and remedy any defects therein and provide all labor,
including the supervision thereof, materials, Consortium's equipment, and all other things, whether
of a temporary or permanent nature, required for such construction.

This HSSE Plan (Health, Safety, Security& Environment Plan) has been developed to provide
guidelines to enable the Consortium and its Subcontractors to plan and assist in effective
management of HSSE requirements by conducting its undertakings in such a manner as to minimize
any potential HSSE effect during project execution in Lumut Balai2 Project.

This HSSE Plan is a package of HSSE documents, including this document and other relevant
procedures and sub-plans. Other procedures and sub-plans are the appendixes of this document.
Refer to the following picture to see the structure of Consortium HSSE Plan. Refer to Appendix 1
for the full HSSE Plan Documents List. This HSSE Plan is a 'live document' as work progresses, the
plan shall be further developed and elaborated which shall be presented to Client for review and
update as necessary.

DEFINITION

HSSE Management System

An integral part of the overall management system in order to manage and control HSSE aspects
related to project activities in order to create the workplace and environment that is safe, efficient
and productive.

Hazard

Source/material, situation or action with the potential to injure in terms of human injury or illness, or
a combination of both.

Hazard Identification, Risk Assessment and Risk control

A sequence of activities to identify hazards, analyze risks arising from hazards and recommend risk
control action.

Risk

Combination of a likelihood of an occurance of a hazardous event or exposure and the severity of


injury or ill health that can be caused by the event or exposure.

Unsafe Act

Actions or malicious behavior of an employee who has the potential to lead to the occurrence of
incidents and accidents.

Unsafe Condition

A physical/environment condition (situation/circumstances) that dangerous and can lead to incidents


and accidents.

Nearmiss

An event that almost cause/nearly injury/damage and if no preventive measures could potentially
cause an accident.

Incident

Event related Work in which an injury or ill health (regardless of severity) or fatality occurred, or
could have occurred.

Accident

An unwanted incidents that cause death, illness, injury, damage and/or other losses.

Emergency

An extraordinary event (earthquake, flood, gas leaks, fires, explosions, demonstrations, and others)
that have the potential hazards in the location of the work project that can be addressed by
applicable emergency procedure in the project work sites.

System LOTO (Lock-out Tag-out)

A safety system in works by isolating the source of energy by providing certain locking and tagging.

MSDS (Material Safety Data Sheet)

An information sheet concerning the properties of the chemicals, the things that need to be
considered in the use of chemicals, help in the event of incidents and accidents, as well as the
management and handling of chemicals hazardous.

Subcontractor

Partners who provide services for Consortium to the construction Lumut Balai2 Project.

Site

Work location of Lumut Balai2 Project, including temporary facilities (workshops, mosques, lay-down
area, the area of accommodation, canteen etc).

Environment

Circumstances surrounding the project site, including air, water, land, natural resources, flora, fauna,
humans and their interactions.

Pollution

Login or inclusion of living things, matter, energy, and/or other components in the water, air and soil.
Pollution can also mean a change order (composition) of water, air and soil by natural human
activities and the project activity, beyond the carrying capacity of the environment so that the quality
of the water/air/soil is reduced and not in accordance with the designation.

Environmental Aspect

Element of activities or the products or services that could interact with the environment.

Environmental Impact

Any change on the environment, whether adverse or beneficial, wholly or partially caused by the
project development activity of Lumut Balai2 Project.

1 LEADERSHIP AND ACCOUNTABILITY

1.1 Management Involvement In Promoting HSSE

Leadership and management commitment of Consortium will ensure that the HSSE Management
System implementation is well done and continuing with the aim to prevent incidents and accidents,
minimize the risk, by prioritizing the management of HSSE aspects in every stage of the project
activities.

The entire management of Consortium is committed to conducting the project activity by giving first
priority to safety and health of Consortium workers, PGE and the general public as well as
environmental conservation in each stage activities of the project development of Lumut Balai2
Project.

Commitment of Consortium leaders management on the implementation of HSSE aspects is a key


to success over HSSE Management System implemented at the project of Lumut Balai2 Project.

A more detailed explanation can be found in the Management Involvement Procedure.

1.1.1 Management Involvement in HSSE Campaign/ Training Program

Consortium project management members shall participate in HSSE leadership training in the
beginning of the project and receive refresh training annually.

HSSE Campaign shall be conducted semi-annually with Consortium project management


involvement, such as safety awareness improvement campaign, fire prevention campaign, etc.

The weekly Master Tool Box Talk shall be conducted and Consortium project management shall be
attending.

1.1.2 Management Involvement in HSSE Meeting

The aspect of HSSE shall be discussed in regular meeting, such as management meeting, progress
meeting and kick-off meeting, etc.

HSSE coordination meeting shall be conducted weekly with participation of Consortium


management and HSSE.

1.1.3 Management Involvement in HSSE Walk Through

Regular Weekly HSE walk through shall be conducted every week with participation of at least one
member of Consortium management, construction representative, HSSE professional.

Management Walk Through shall be conducted quarterly with participation of PGE, Consortium and
Subcontractors management.

1.1.4 Intervention on Substandard Conditions and Behavior

Substandard conditions and behavior shall be corrected immediately in site if it is safe and practical
to do so. Short period shall be given to take corrective action if it takes longer time to do so, and
remedial actions shall be followed up by HSSE team. The work shall be stopped if there is imminent
danger or serious violation.

Consortium wishes to emphasize the Stop Work Action Policy that everyone has the right and the
duty to stop work if it is concluded that the work cannot be done in full safety. No penalty is attached
to stopping unsafe acts or situations.

There is HSE Golden Rules and Life Saving Rules from Pertamina. It is mandatory that work
environments enable compliance with these rules. Everybody acting in this project must be
committed to enforce them and to respect them.

1.2 Rewards and Consequences

1.2.1 Reward system

The purpose of the HSE reward system is to influence all employees to participate in the HSE
system and to believe that HSE is a value and not just a priority and to influence the behavior of
individuals and groups and develop a project safety culture.

This HSE award is given to best workers, best work group and best parties every month based on
the worker behavior and HSE performance evaluation.

Other awards programs such as award for the achievement of safe man-hour during the project
activity held, given every 500,000 hours worked safely in the form of celebration or a certificate.

1.2.2 Sanction System

If the workers doing violation of projects regulation include HSSE aspects will be subject to strict
sanction until termination of employment from the Lumut Balai2 Project. The worker shall be warned
and penalized if he is committed to violation as 1st time warning, 2nd time retraining and suspension
from work for some time, 3rd time termination from site.

Violence to any person or group shall not be tolerated and is subject to strict disciplinary action
which may ultimately lead to termination from the site. Any form of racism or religious fanatical act
which is intimidating to others at the project site is considered as a major violation which shall not
be tolerated and is subject to strict disciplinary action.

2 POLICY AND TARGET

2.1 HSSE Objective and Policy

2.1.1 HSSE Policy

Leaders and all employees of Consortium are committed to create a safe and comfortable workplace
by eliminating potential hazards and reducing occupational health, safety, and environmental risks
in order to achieve the target of zero fatality without occupational disease, and environmental
damage, by:

1. Implement Occupational Health, Safety, Security and Environment Management System in


accordance with the legislation requirements, applicable national and international standards and
Contract HSSE Requirements;

2. Improve Occupational Health, Safety, and Environment expertise and competencies for
employees;

3. Encourage consultation and participation the implementation of the Occupational Health, Safety,
Security and Environment Management System.

Consortium is continually improving its Occupational Health, Safety, Security and Environment
Management System on an ongoing basis through the learning and best practices of the

Management System at the national and international levels and keeping abreast of the latest
knowledge in the Occupational Health, Safety, and Environment field.

2.1.2 Alcohol, Drug and HIV Prevention Policy

Leaders of Consortium are committed to ensuring a safe, healthy and productive work environment
while protecting the general public, property and company assets.

1. Not tolerating employees and work partners who possess, keep, consume and/or distribute
alcoholic drink and/or illegal drugs in all of the Company's work units;

2. Do random checks. lf proven positive for consuming alcohol and illegal drugs in all work units of
the Company, then the employee or work partner will be subject to sanctions or Termination of
Employment and reported to the authorities;

3. Encouraging workers to carry out routine, confidential and voluntary HIV/AIDS tests accompanied
by counseling;

4. Facilitating employees who suffer from HIV/AIDS and protection from discriminatory acts and
treatment in accordance with applicable national regulations;

5. Keep all medical information, health records or other related information confidential.

6. Prohibit the use of illegal drugs, alcohol, use of firearms, gambling and fighting

This policy must be communicated to all employees and work partners so that they are complied
with and implemented to realize a better quality of life.

2.1.3 Stop Work Action Policy

Management of Consortium gives full authority to every employee & partner to prevent work
accidents by:

1. Stop Work Action (SWA), if there is any unsafe action/condition or work which does not comply
with the applicable regulations;

2. Share the SWA experiences which one does or qiven at the safety meeting.

Management guarantees that the SWA initiator will not be subject to any sanctions even though the
termination will cause a delay in the completion of the work.

On the contrary, a sanction will be given to employees who ignore SWA they have received or have
not done SWA when they see or know of unsafe action/condition.

2.2 HSSE Performance Target

The specific HSE Performance Target are indicated by HSSE Performance Indicator.

Consortium will collect Lagging & Leading indicators data as HSE performance benchmarks at
Lumut Balai2 Project.

3 ORGANIZATION, RESPONSIBILITY, RESOURCE, AND DOCUMENTS

3.1 Organizational Structure, Duty and Responsibility

3.1.1 Organization Chart

3.1.2 Responsibility

HSE in the work place is everybody’s responsibility, and certain individuals will be required to accept
additional responsibilities based on job assignment.

These will include the following:

a) Project manager

 Has the overall accountability for construction site HSE.

 Allocate and ensure adequate resources are available for the SITE HSE implementation.

 Be accountable for achieving HSE goals.

 Be responsible for ensuring that Subcontractors implement, administer, plan, train, and
enforce the HSE plan.

 Establish the site HSE and Security Plan and be responsible for its implementation before
starting site activities.

 Use only competent personnel to work on site.

 Issue written instructions setting out the method of healthy and safe work in accordance with
the policy on high-risk activities where necessary.

 Plan and maintain housekeeping to a high standard.

 Inform the management of Contractor and also CLIENT Representative(s) of any accidents,
incidents, and mishaps with the potential of injury and illness consequences immediately.

 Encourage the reporting by all persons of 'near miss incidents'.

 Arrange for thorough investigations of all ‘near miss incidents’ with lessons learned and
openly publicized to all persons concerned.

 Cease all activities in the area of an identified health and safety problem until it is resolved.

 Immediately remove from the site any person (staff, subcontractor or third party) who is not
willing to comply with the HSE requirements.

 Provide HSE training opportunity to all employees.

 Perform corrective actions for any identified unsafe conditions.

 Be responsible for setting up appropriate training, including refresher training as needed, to


enable managers and workers to assess hazards, and to familiarize themselves with the
relevant requirements (legislations, regulations and Client standards).

 Check working methods and precautions for high risk activities with HSE Manager before
work starts and in accordance with the HSE policy.

b) Site HSE manager

 Assist the Project Manager in the management and execution of the HSE Plan.

 Conduct regular site inspections and prepare reports for Project Manager for corrective
actions.

 Check and advice on each work plan or procedure for HSE prior to work commencement.

 Coordinate with the CLIENT’S section on matters regarding HSE.

 Liaise with the Subcontractors’ HSE Managers on matters regarding the health and safety of
all workers under the Subcontractors.

 Act in the capacity of Secretary to the HSE Committee.

 Provide advice, guidance and such, as may be needed in accident prevention.

 Coordinate with Government Authorities on matters pertaining to legal HSE requirements.

 Establish and maintain an adequate HSE administrative system.

 Investigate all near misses, incidents and accidents and submit reports complete with
remedial actions to the HSE team of Contractor’s head office through Project Manager for
corrective action.

 Ensure that Safety supervisors conduct their safety activities in a professional manner.

 If HSE violations are determined to be high risk, then stop the work as necessary to co-
ordinate and monitor implementation of the requirements of the Client’s HSE requirements
and arrange continuous and formal daily safety monitoring to ensure compliance and
effectiveness.

 Prepare regular toolbox safety briefs and monitor safety information.

 Review completed risk assessments and the monitor implementation.

c) HSE engineer

 Provide timely communications on significant issues or developments.

 Coordinate HSE issues between different Subcontractors.

 Conduct site inspection of safety facilities.

 Inspect the workers construction environment/situation.

 Inspect safety work of safety staff.

 Compile a summary of the previous day’s safety work.

 Prepare a safety plan for the following week.

 Organize regular safety meetings with subcontractors.

 Carry out special inspections at least two times a week.

 Prepare a monthly summary of the safety work.

 Prepare a monthly plan of the safety work.

 Take part in monthly inspection of HSE work.

 Take part in monthly HSE analysis meeting of subcontractors.

 Conduct HSE training as per the training plan.

 Audit the work load of subcontractors.

 Check the relevant safety measures and safety work permits.

 Handle all correspondences with subcontractors.

 Handle and analyze safety accidents.

 Make the necessary arrangements for safety meetings.

d) Safety supervisor/officer

 Assist HSE Manager and perform the safety activities under the direction of the HSE Manager

 Check each subcontractor’s work plan or work procedures from safety angle and report to
the HSE Manager.

 Advise supervisory personnel on the related HSE activities of each function.

 Patrol the construction site daily to check that all subcontractors' works are executed in
accordance with the HSE Procedures.

 Report immediately to the HSE manager of any near miss incidents or safety violations which
may require work to be stopped.

 Contractor’s field supervisors are responsible for ensuring effective implementation of the
Safety Management Plan in all areas under their direct control.

 Anticipate HSE issues in their respective work areas and take the necessary actions.

 Report any incident/accident, unsafe acts/conditions and near misses to their HSE officer.

 To ensure work places are kept clean during and at the end of the work.

 To carry out or support team talks prior to the work, inform the work crews of the requirements
of the Permit- to-Work Certificates & Risk Assessments.

e) Medical Staff

 Be on duty at the job site during working hours (except when on emergency calls). Care for
an injured or sick person and keep the record.

 Hold valid certification in such as, ACLS(Doctor), ATLS(Doctor), BTLS(Nurse) and PPGD,
and Company Hygiene and Occupational Health (Hiperkes) certificates.

 Plan and execute the hygiene activity program for all workers at the work site.

 Prepare a written daily report of all accident/medical treatments provided.

 Prepare a monthly report listing all types of medical/accident treatments.

 Reports to be made available at all times to Contractor HSE manager and other interested
parties.

 Nurse shall conduct the necessary medical examination.

f) Trainer

 Conduct HSSE Induction to each employee and refreshment training.

 Conduct specific training as training need matrix and training plan, such as working at height,
hot work, H2S prevention, safe driving, lifting, etc.

 HSSE document control, such as update meeting minutes, walk through report, HSE report,
etc.

 Communication to subcontractors and client.

g) Subcontractor’s construction manager

 Provide a HSE Plan that shall at least be comparable to this document.

 Ensure good HSE discipline by all his personnel.

 Participate in Contractor HSE Committee.

 Ensure that all levels of supervisors are qualified with appropriate trainings.

 Encourage the reporting by all persons of 'near miss incidents'

 Arrange for thorough investigations to be made of all 'near miss incidents' with lessons
learned openly publicized to all persons concerned.

h) Subcontractor’s HSE manager

 Assist his Construction Manager in executing and enforcing the WORK in accordance with
the approved HSE Plan.

 Conduct regular checks of the work areas under his jurisdiction for safety violations, unsafe
conditions, defective equipment, materials, tools, apparatus and appliances and shall take
immediate corrective actions.

 Generally oversee that the work performed by workers under his jurisdiction.

 Initiate and assist supervisory personnel in conducting their daily Tool Box Talks.

 Organize periodic HSE Meetings.

 Organize and conduct regular HSE promotion activities with the aim of creating HSE
awareness among workers.

 Establish accident prevention programs, review reports and participate in discussion on near
misses, dangerous occurrences, injury, illness and property damage incidents.

 Foster good HSE working relationship among Subcontractors.

 Keep and maintain all HSE records pertaining to the SITE HSE programs.

i) Subcontractor’s supervisor

 Participate in the work site HSE programs, especially on safe work procedures with the aim
of preventing accidents.

 Coordinate all work activities to ensure that they meet the required standards with minimum
risk to person, equipment and property.

 Conduct regular checks at the site for safety violations, unsafe conditions, defective
equipment, materials, tools, apparatus and appliances and take immediate and appropriate
actions to eliminate the hazards.

 Submit written reports with recommendations for corrective actions to prevent recurrences of
any property damage or injury accident at the job site. The reports shall be submitted to the
HSE Manager.

 Submit written reports of any other incidents, violations and unsafe work conditions at the job
site. The reports shall be submitted to the HSE Manager.

 Encourage the reporting by all persons of 'near miss incidents'

 Arrange for thorough investigations to be made of all 'near miss incidents' with lessons
learned openly publicized to all persons concerned.

 Ensure all HSE work instructions are clearly understood by the workers.

 Conduct regular talks to promote HSE awareness among the workmen.

 Conduct daily Tool Box Meetings and highlight potential hazards to the workers under his
jurisdiction.

 Set a good HSE example.

j) Employee (Worker)

 Comply with HSE rules and regulations.

 Work safely and shall not do anything that can cause injury to himself /herself or others.

 Ensure work-tools, apparatus, appliances, materials, and/or equipment including Personal


Protective Equipment (PPE) are used correctly and maintained in good serviceable
conditions.

 Report any unusual occurrences and all defects of plant and equipment to your immediate
supervisor.

 Report all near misses, incidents and accidents to immediate supervisor.

 Attend all Tool Box Meetings, other meetings and/or training relating to HSE.

 Observe all written and verbal HSE instructions issued from time to time by the HSE Manager
and/or HSE Supervisor.

 Observe and obey all HSE signs/notices.

 Develop a personal concern for the HSE.

 Keep work place clean and tidy.

 Seek medical assistance for all injuries.

 Report to the medical center if feeling unwell.

3.1.3 HSSE Personnel

Consortium will appoint an HSSE Manager to ensure the process of implementing the HSSE aspect
system & procedure in the running project.

Sufficient numbers of competent HSSE Project Coordinator and HSSE Officer will be mobilized
accordingly as required by EPC Contract. And HSSE professional is required when Contractor and
Subcontractor employees reach 50 people and another HSSE professional is required if the number
of employees reaches 100 and so on.

The HSSE professional shall be reviewed and approved by Client.

3.2 Medical Examination

All Consortium workers and subcontractors who worked at the job site must perform the Medical
Check Up and get reports of medical records in a healthy state (Fit To Work) by doctor.

Workers with work contract ≤ 3 months and/or work implementation period ≤ 3 months provided
that:

They have a HEALTH certificate issued by a health center doctor, clinic, or hospital who is legally
allowed to carry out health checks and issue a health certificate. The health certificate is valid for a
maximum of 6 months from the date of examination.

Workers with a work contract and work execution period > 3 months with the following minimum
inspection standards:

The minimum standard of inspection as shown in table 1 above is valid for a maximum of 1 year.

3.3 Employee Insurance

All site employees shall be covered by employment insurance program following Law Number 24 of
2011 concerning the Social Security Administrator.

3.4 Training and Competency

HSE training is essential for executing work safely during the entire construction, pre-commissioning
and commissioning of the project. HSE Training Program includes the following five categories:

 HSE Induction Training for New Personnel.

 HSE Training for Specific Hazardous Job, such as working at height, hot work, h2s prevention,
confined space, etc.

 Emergency and Evacuation Training.

 HSE Training Refresher course.

 HSE Tool box talks.

 HSE training for Visitors.

3.5 HSSE Communication

HSE Communications are conducted to ensure the success of communication between different
parties and employees with the participation of different parties and employees so that the
information in question can be understood by both parties and employees.

3.5.1 Daily Tool Box Meeting

This meeting is done every day before doing activities. Intended to encourage employees to identify
hazards and analyze the risks that they know and how they can perform tasks at each stage of their
jobs more safely. The meeting also discussed the previous program and future plans.

3.5.2 Weekly Master Tool Box Meeting

This meeting is done on the first day of every week with participation of all project members and
Consortium Management.

3.5.3 Weekly HSE Coordination Meeting

This meeting is intended to control all HSE aspects of project activity weekly to ensure and meet
good HSE performance without any accidents.

3.5.4 Construction Coordination Meeting

This meeting to address any problems that are detected from each stage of EPC ranging from
engineering, mobilization, procurement, construction and commissioning, to review the
implementation of HSE aspects of each construction activity of subcontractor to achieve the project
activity so the accident/incident can be minimized.

3.5.5 HSE Committee Meeting

Consortium recognizes the need for the HSE committee meeting to discuss and resolve the concern
findings relating to health, safety and environment. Consortium will hold a HSE Committee meeting
quarterly.

3.5.6 HSSE Campaign

Consortium shall conduct quarterly HSSE Campaign to help employee to navigate the HSE and
improve their health, safety and wellbeing. Consortium management shall attend the HSE campaign.

3.5.7 HSSE Board and Sign

HSSE board shall be installed in office and site, such as HSSE Performance KPI board, HSSE
bulletin board, Poster board, etc. Other warning signs shall be posted in site to indicate the hazard
and safety regulations.

3.5.8 Lesson Learnt

Consortium shall provide lessons learned to encourage continuous improvement in the performance
of HSSE. All Consortium workers shall be encouraged to report near misses, unsafe acts, and
unsafe conditions that offer the greatest learning opportunities. Master lesson learnt shall be
conducted after incident investigation.

4 RISK MANAGEMENT

Risk management systematic is to be done in order to ensure that each project activity can be
carried out safely, reliably, and friendly environment so that undesirable events can be minimized.

Consortium and Subcontractor shall compile Risk Register Document for Contract work which is
with a contract work duration of more than 6 months. The potential hazards and mitigations listed in
the Risk Register Document must be implemented during the implementation of the contract work
to prevent incidents.

Then the Consortium and Subcontractor shall compile Job Health Safety Environment Analysis
(JHSEA) based on the prepared Risk Register Document to ensure the potential hazards of each
activity to be carried out have been identified and appropriate mitigation has been established.

The compiled JHSEA document must include identification and analysis of potential hazards and
mitigation plans that will be carried out starting from the pre-mobilization stage, mobilization,
implementation of work to demobilization.

The Risk Register Document and JHSEA document shall be communicated to workers and
socialized through Training, Tool Box Talk, etc.

5 PLANNING AND PROCEDURES

5.1 Work Procedure and Safety Standards

Consortium shall develop work procedures that regulate safe working instructions which include but
are not limited to:

1) Safe work methods and equipment handling.

2) Safe Work Practices for work activities with critical potential hazards under Pertamina's Corporate
Life-Saving Rules (CSLR).

3) Equipment inspection and/or certification to ensure that the equipment used can be operated
safely and complies with regulatory requirements.

4) Qualification requirements for Operators who operate equipment according to the type and
regulations that govern it.

5) Applicable safety standards.

6) Intervention on the behavior and conditions of the Sub Standard that occurs.

The work procedure is usually in the form of work procedure and method statement.

The typical examples of work procedures are as following:

5.1.1 Working at Height

Consortium will make the principles and procedures for working at height with control methods of
work to ensure the safety and welfare of all workers. The general contents of the Working at Height
include:

1) Minimize the Working at Height activities: Avoid work at height where we can; If the avoidance of
work at height is not possible, use work equipment or other measures to prevent falls where they
cannot avoid working at height; Where they cannot eliminate the risk of a fall,
use work equipment or other measures to minimize the distance and consequences of a fall should
one occur.

2) All personnel involved with work at height is declared medically fit and had undergone training
work at height is carried out by Consortium.

3) Implement Permit to Work system for working at height.

4) Use proper and safe access, scaffolding, ladders and working platform for the workers to work at
height. Implement the fall prevention safety measures, such as guard rail system, tore board,
protection for manholes, safe scaffolding, etc. All the scaffoldings shall be inspected and tagged.

5) Provide fall arrest/protection system for the workers, such as full body safety harness using, safety
net, life line system, restriction line, etc.

6) Prepare emergency rescue plan for working at height and conduct simulation.

5.1.2 Confined Space Entry

Consortium will make the principles and procedures for Confined Space Entry with control methods
of work to ensure the safety and welfare of all workers. The general contents of the Confined Space
Entry include:

1) Identify the confined space in site, put proper barrier or gate to close the access of confined space,
put confined space sign on the entrance.

2) Implement Permit to Work system on Confined Space Entry.

3) All the workers enter the confined space shall undergo Confined Space Training.

4) The confined space shall be checked and tested to ensure it is safe to perform work inside it.
Such as gas test and gas monitoring of confined space. Provide safe access and egress for confined
space. Provide ventilation for confined space. Provide sufficient lighting for confined space. Provide
safe power supply for confined space.

5) Assign confined space watcher to secure the confined space, standby to record all the workers
and response to emergency cases.

6) Prepare confined space rescue plan and conduct simulation.

5.1.3 Other Work Procedures

Other work procedures/ Method Statement shall be submitted to PGE for approval prior the exact
task starts.

5.2 Work Procedure Socialization

Work procedure, Method statement shall be communicated to workers through meeting, training,
tool box talk, etc. The copy of work procedure and method statement in English and Indonesia
language shall be available in site for workers and supervisors to use.

5.3 Emergency Response

5.3.1 Emergency Plan

Consortium prepares an emergency response plan for the contract work to be carried out. And
specific emergency cases shall be discussed and response- prepared, including but not limited to:

1) Worker at height rescue.

2) Fire-fighting.

3) Confined space rescue, H2S rescue.

4) Personal injury.

5) Electrical shock.

6) Collapse of scaffolding, structure or excavation.

7) Chemical Spillage.

8) Response to Extreme weather event such as flood, storm, etc.

5.3.3 Medical Team and First Aid

Consortium shall build one project clinic with clinic facility, medical treatment equipment, medicine,
doctor, nurse and ambulance as the specification required in the EPC contract.

As project clinic will be based in the camp/ office area, emergency first aid if required in the work
location. Sufficient First Aider and first aid equipment shall be provided in the work location to provide
the necessary immediate first aid.

5.4 Management of Pandemic Spread

As Covid-19 is still spreading during the project execution, it is critical for consortium to prepare and
implement Covid-19 prevention plan. The general arrangements shall be as following:

1) All consortium workers and employee entry site must have been vaccinated against covid-19 with
3rd booster. For foreign employee, the requirements will be according to the regulation from the
country. For those who cannot be given covid-19 vaccine due to medical reason should give the
evidence.

2) Implement preventive measures, such as disinfection of public area, ventilation, medical


monitoring, etc.

3) Implement quarantine, medical treatment measures as lasts guidance of Ministry of Health


Republic Indonesia.

6 IMPLEMENTATION AND OPERATIONAL CONTROL

6.1 Management of Change

The changes in the related personnel, equipment used and processes/methods carried out shall be
managed through Consortium Manage of Change Procedure.

6.2 Equipment Inspection

The equipment used for site work shall be inspected prior entering site by joint team of PGE and
Consortium and regularly inspected by Consortium team.

1) The inspection shall be conducted based on the equipment inspection check list.

2) The general function and safety device shall be tested during the inspection.

3) The required certificate from authority or 3rd party shall be verified.

4) Regular inspection shall be conducted by operator, machinery supervisor in site.

6.3 Permit to Work system

Permits to work procedures are necessary for performing work safely in high hazard areas.

During construction, the following work permit system will be used, but not limited to:

 Working at height permit.

 Excavation work permit.

 Radiation work permit.

 Confined space entry permit.

 Heavy lifting permit.

 Lock out/Tag Out permit.

 Others.

The permit to work is a documented tool to ensure that all the hazards are identified, and the
necessary precautions are taken to eliminate or isolate the hazards of the work to be performed.

Some work may require only one type of permit, while other types of work may require a combination
of permits to ensure that all hazards are identified.

A completed review of the permit-to-work system will take place prior to the plant becoming live.

6.4 Subcontractor Management

6.4.1 Subcontractor Selection

It is contract requirements to select a Subcontractor on the basis of their HSSE competence.


Evaluating the Subcontractor qualification is the most important part of this process. And this is
where the qualification status of the contractor is determined.

Prior to the bidding stage, an Evaluation Form will be issued to all prospective Sub-contractor to
determine the level of HSSE safety competence that the applicant has in place. The applicants must
complete this form and where requested provide the corresponding documents before being
considered for entry to the bidding stages.

Only the Subcontractor that has submitted all information as requested which is complete and has
been assessed as “Satisfactory” can be selected as qualified Subcontractor.

6.4.2 Subcontractor Management in Execution

Subcontractor shall implement HSSE Management System as specified in the HSSE Plan.
Consortium will check the arrangements that Subcontractors have in place for managing the HSSE
of their employees and protection of other persons not involved in their activities. Subcontractors
are subject to evaluation on their HSSE performance and Audit on HSSE Management System.

7 ASSURANCE MONITORING, MEASUREMENT AND AUDIT

7.1 Inspection

Regular Weekly HSSE walk through (Inspection) shall be conducted every week with participation
of at least one member of Consortium management, construction representative and HSSE staff.
The camp/ office/ storage area shall be also covered in the HSSE walk through scope. As the site
work, camp, office and storage area are located in different places, so a specific area shall be
focused during one walk through. A notification shall be sent to relevant parties 3 days prior walk
through.

Management Walk Through shall be conducted quarterly with participation of PGE, Consortium and
Subcontractors management.

All observations made during the HSSE Walk Through shall be recorded. Copies of the report shall
be distributed to all parties concerned for corrective actions.

HSSE Professional shall conduct follow-up actions to ensure that corrective actions on all HSSE
Observations are carried out satisfactorily.

7.2 Monitoring

The HSSE Performance of Subcontractors shall be monitored by Consortium, and this is usually
performed through HSSE Report with HSE KPI including Daily HSSE Report, Weekly HSSE Report
and Monthly HSSE Report. Beside HSSE Report, the HSSE Performance of Consortium and
Subcontractors shall be evaluated every month based on PGE HSSE Performance Indicator
Evaluation Form.

7.3 Audit

Internal HSSE audits will be conducted quarterly by Consortium to verify compliance with HSSE
Plan, where the HSE audit activities should be known and monitored by the HSSE PGE. Consortium
HSE Manager has the responsibility to ensure the Internal HSSE audit done on schedule. The HSE
audit results will provide a clearer picture about the achievement of the program has been
implemented and analyzed a variety of circumstances faced by the appropriate application of HSE
aspects.

Beside Consortium Internal HSSE Audit, there will be Audit from PGE and Consortium Head Office
Audit.

7.3.1 Incident Reporting and Investigation

Suppose an incident occurs in site, consortium shall report to PGE immediately by phone, Whatsapp
or other apps within 1 hour. And an initial report shall be submitted to PGE within (6) hours of the
occurrence of the incident.

Detailed full investigation shall be conducted by Consortium within 7 days after the incident if
required. The relevant incident investigation report with root cause analysis shall be submitted to
PGE if required.

PGE shall report to local authority if it is required to do so by law.

8 REVIEW

HSSE Management review shall be conducted during the execution of contract work involving the
Management level semi-annually. The review is carried out to evaluate and make continuous
improvements to the implementation of the HSSE Plan, which includes but is not limited to the
following:

 HSSE risk management evaluation,

 Proactive efforts to implement HSSE,

 The suitability, adequacy, and effectiveness of the implementation of the HSSE Plan,

 Achievement of HSSE performance during the execution of Contract work,

 Competent allocation of resources and their adequacy,

 incident happened,

 Findings and follow-up on audit and/or inspection results.

The management review activities shall be documented. The finding of review shall be recorded
and followed up by HSSE team.
