The humble petition of the petitioner

above named most respectfully –

S H E W E T H:

1. Your petitioner is a law abiding citizens of India and permanently reside

at the address mentioned in the cause title.

2. Your petitioner was arrested on ……………… in respect of the Jorasanko

Police Station Case No…………….. dated ……………..under Sections 420/120

B and added Section 409 of the India Penal Code and was thereafter released

on bail after custody of 87 days on ……………..by this Learned Court.

3. Your petitioner states that one ……………..lodged a First Information

Report with the Office-in-Charge of the Jorasanko Police Station being

P.S. Case No. …………of 20 dated ………….and a case was started under

sections 420 and 120B of the Indian Penal Code.

4. Your petitioner states that thereafter one application for bail was moved

before the this Learned Court and after hearing both the parties Your

Honour by an order dated ………….was pleased to allow the prayer for

bail subject to the condition that the petitioner shall meet the

Investigating Officer of the case twice in a week until further orders.

A photocopy of the order dated ………………..is annexed hereto and

marked as Annexure "P-1".

5. Your petitioner state that since your petitioner has been granted bail by

this Learned Court, the petitioner herein are abiding by the condition

imposed upon him and never violated the same.

6. Your petitioner has been meeting the Investigating Officer of the case

twice every week as directed by this Learned Court.

Photocopies of the endorsement of his appearance before the

investigating Officer are annexed hereto and marked as Annexure "P-2”.

7. Your petitioner states that the petitioner is a senior citizen and is

suffering from severe cervical spondilytis and is under medical treatment

for the same and in view of the above it is submitted that the condition

as imposed by this Learned Court by an order dated …………………..in

may be waived and/or relaxed for the ends of justice.

Photocopies of the relevant medical records are annexed hereto and

marked as Annexure “P-3”.

8. Your petitioner submit that your petitioner is a respectable citizen in his

locality and have been strictly abiding by the condition as imposed upon

him by this Learned Court at the time of allowing his prayer for bail.

In view as aforesaid,
