ADMISSION FORM
(Sindh MBBS and BDS)

 Hello Everyone!

This is Dinesh Kumar, recently a first year MBBS student

at Dow Medical College, and also a Campus Ambassador
Head at PreMed.PK.

Since you guys are done with your MDCAT and now in the clueless
phase of documents gathering, Team PreMed.PK is to the rescue 🎉
Let's get started!
There are two forms, one online and one physical, separate for
MBBS and BDS. Both are supposed to be filled for getting into any
government MBBS or BDS university. In case you are not sure about
your merit number and are willing to apply for BDS, then fill the BDS
form additionally too(for the safe side). The fees, 2500PKR, will be
paid for them individually and the Challan is supposed to be
attached with the physical form. And there are particular TCS
mentioned in the prospectus which will take these forms and
submit it by themselves. (TCS fee: 350Rs per form).

Stay careful while selecting the form. If applying for MBBS
then both forms must be of MBBS (1&3) and if BDS (2&4). DO
NOT make mistakes in selecting the forms!!
1st form is online MBBS, 2nd form is online BDS, 3rd one is
physical MBBS, 4th one is physical BDS.
Also, have screenshots of your online filled form for the safe
side and cross-check it while filling the physical form, because
information should NOT differ.
The number in your BForm is the same as your CNIC number.
Deadline: 11th Dec/2023.

Online Form:
Fill your personal details here.
Provide identical information in both your online and physical
forms, ensuring consistency in personal details. Follow the
instructions outlined in the physical form to accurately fill in
your online details.
Add your particulars of MDCAT
Add guardian/parents details
Fill your emergency contact details.

 Add your academic details.
Choose your preffered medical colleges in the following
sequence: DUHS, JSMU, KMDC, SMBBC-Lyari.
If the online form requests it, please upload PDFs of your MDCAT
scorecard, as well as the original IBCC attested SSC and HSC
marksheets.

Note
Review each page of your online form thoroughly before
progressing to the next one.
It is mandatory to fill online form before filling the hard copy
physical form.
Once you're done with the online form, take pictures of each
page. This will help you check and compare the information
needed for the physical form later on.

Fill your details in BLOCK (capital) letters
Name, Fathername, Surname (if any) should be the one on your
B-Form/CNIC.
In the top box, state your district. If you live in Lyari Town, Bin
Qasim Town, Kemari Town, or Gadap Town, mention the
specific town.
For Karachi residents in other areas, simply write Rest of All
Towns, Karachi.
Your District is mentioned in domicile and PRC.
If anyone does not have their Domicile or PRC issued, go to DC
office with documents required (photographs, matric result,
Bform/CNIC)
Add your functional email which you check regularly.
Test Center: Expo-Karachi
For the matric and intermediate column, you have to add your
RollNo/CandidateNo in the seat number column. (Class 10th and
Class 12th)
Passing year is the year of exams for class 10 and 12th.
Name of board: BIEK/BSEK or AKU-EB or FBISE, etc.
80+ is grade A1, 70+ is grade A and so on.
Date of submission is the date when you are going to courier it.

Fill in with your same details for 8 times

Present address is the address in which you are living right now.

Fill your Father’s/Guardian’s details.

Present address is their current address, and permanent address
is the address written on their PRC.
Emergency contact can be anyone. E.g. your mother, brother,
sister, uncle, etc.
No matter where your merit no stands, you can prioritize as you
want. Even if you have an aggregate which you feel won't be
sufficient for a university, you can still prioritize it first.
DOW, JSMU, KMDC or SMBBMCL, LUMHS, PUMHSW (if female),
etc (Reciprocals seats should be mentioned if you want to take
admissions in them)
(God forbid) If Disabled then add a check(✓) in Yes column (they
will check the certificate of disability else tick it in No blank.

Collect all the attested copies of documents mentioned. Not the

original ones!

Put a in every box once you collect them.
Arrange all those, including the form, in the sequence
mentioned in the checklist.

Let's say you are applying for MBBS. You fill two physical forms
for MBBS(can send upto three too, so that if one is misplaced
so you have something on the safe side. So you have to send
double copies set (two sets for one programme), one set with
each form, but pay the fee only once and have the voucher
photocopied).
So if applying for MBBS and BDS both, a total of 4 sets of
documents will be there (2 for MBBS and 2 for BDS).
Pay the fees of 2500 Rs. for each form at the TCS centre and
attach the paid receipt provided by TCS, with the form before
submitting. If possible, consider photocopying the receipt and
keep it eith you as a proof of payment.
TCS charges for form delivery are 350 Rs each.
Photocopies of all documents and Candidate's Pictures should
be attested. Attestation can be done by any 18th grade+ govt
officer or from your colleges too.
Keep contacting your schools or board office for the
Matriculation certificate.
