Usability Test for Andie Dennison’s Print User Manual

Objective: The goal of this usability test is to determine the success of my print user
manual that details the directions of inserting audio to Microsoft PowerPoint slideshows.

This is a consent form. Please read the following information thoroughly and sign this
form:

I agree to participate in an in-person usability test using my own computer. I

understand that during the usability test, I will be asked questions about Microsoft
PowerPoint and asked to insert audio into a slideshow that I create using a
student developed print user guide.

I understand and consent to the use of the results by the VT student researcher. I
understand that the information is for research purposes only and that my name
and image will not be associated with any data collected during this study. I
relinquish rights to any feedback I give during this in-person interview.

I understand that I will need to be in-person on Sunday, October 8th for this
session and that I must bring my own laptop with Microsoft PowerPoint
downloaded. I understand that this session will take roughly fifteen to thirty
minutes.

I understand that participation is voluntary and that I may stop participating in the
test at any time.

By signing this form, I confirm that I have read and understand the information
provided above. I agree to participate in the study and give my consent for the
researcher to use data collected during the study for their class research
purposes.
