AFFIDAVIT OF DAMAGES

I, KRISTINE JOY P. FLORES, married, of legal age, Filipino and

resident of 316 St. Jude St., Holy Family Village, Taguig City, under oath
depose and say that –

1. That I am the authorized driver of that certain motorvehicle –

NISSAN NAVARRA with Plate No. ABT 3123 registered in the
name of Dhanerika D. Manalo.

2. That on September 29, 2018 at about 8:00 pm, I parked the

aforesaid motorvehicle at the parking lot of Paulina Bldg. located at
162 M. Almeda St., Brgy. San Roque, Pateros, Metro Manila and
left the same.

3. That on the morning of September 30, 2018 at about 9:00 am, I

went back to the parking lot of Paulina Bldg. located at 162 M.
Almeda St., Brgy. San Roque, Pateros, Metro Manila, but to my
surprise, I found out that the aforesaid vehicle - NISSAN
NAVARRA with Plate No. ABT 3123 sustained dents and deep
scratches at the back portion near the tail light and across the
body of the aforesaid vehicle and dents and scratches at the front
bumper, probably a clear case of hit and run.

4. That I asked the guard on duty on the whereabouts of the

perpetrator who bumped the parked NISSAN NAVARRA with Plate
No. ABT 3123 but to no avail.

5. That what happened was purely accident and beyond my control

as the authorized driver of the above described motor vehicle on
that day – between 8:00 pm of September 29, 2018 to 9:00 am on
the following day – September 30, 2018.

6. That I am executing this affidavit to attest to the truth of all the

foregoing for all legal intent and purposes specifically for the repair
of the damages caused to the car - NISSAN NAVARRA with Plate
No. ABT 3123 registered in the name of Dhanerika D. Manalo.

IN WITNESS WHEREOF, I have hereunto set my hand this

October 1, 2018 at Pateros, Metro Manila.
