FREESHIP FORM -
 Duly filled application Form Print Out of web portal
 True Copy of Caste Certificate,
 Gap Certificate Original with One True Copy. (If applicable)
 Original Income Certificate
 One True Copy of Leaving Certificate (Only for Newly Admitted
Students )
 One True Copy of 10th, 12th & Mark Sheet of Last Exam.
 If candidate is out of Nagpur in Maharashatra State they should
submit Original Domical certificate. Students who have changed their
domicile need to submit district change Certificate with true copies

Fill up the details in form & upload Attested Scanned

document online.
The True Copy of Documents attached must be attested by
Gazetted Officer only
(Attestation by Sp. Ex. Officer., Corporetors is not
acceptable)

SCHOLARSHIP FORM FOR S.C.& S.T SUDENDTS

 Duly filled application Form Printout of web portal

 True Copy Of Caste Certificate,
 Original Income Certificate ((Only for Newly Admitted Students in
this institute )
 Students whose parent have stationed business or employed in
Stationed business ( for Ex. Kirana Stores, General Stores, any shop
& Private Company) need to submit Form No. 16 & Service
Certificate on employers official letterhead specifying annual
(gross) salary
 Gap Certificate Original with One True Copy. (If applicable)
 Students who had a gap of one or more academic session, need to
submit gap Certificate.
 One True Copy of 10th, 12th, & Mark Sheet of Last Exam.
 One True Copy of Leaving Certificate (Only for Newly Admitted
Students )
  If Candidate is out of Nagpur in Maharashatra State they should
submit Original Domical Certificate. Students who have changed
their domicile need to submit district change Certificate with true

 Students who received scholarship for last session need to provide
that information & Attach Mark-sheets true copy of last exam of Ist
or II nd year .
 One True Copy of Mark Sheet of Last Exam.

*NOTE :–
* Keep one portal print copy of filled up form for
future reference.
* Kindly understand that above documents are asked by
Social Welfare Office towards completion of documentation.
DAIMSR is only acting as an advisor and facilitator for the
students. If the form is rejected because of documents, the
students alone will be completely responsible for the loss.
* If students fail to register online & submit the Scholarship &
Free ship forms by 31/01/2022, he/she will have to pay the full fees
of the respective course.
