Subject:- Accident Information and Injury details.

Dear Sir/Madam,

As per the above subject, on 12th may 2023 my mother Mrs Saraswathi Y Bat
travelled to Yellapur, Karnataka to attend one of relative’s marriage function.

While coming back she met with car accident (KA 47 M 9223) and Car Owner
& Mr Narayan Ramakrishna Hegde was driving the car.

Please note that Car Driver/Owner Mr Narayan Ramakrishna Hegde was

covered the Car along with Co Passengers with Liberty Insurance (Insurance
Copy Attached).

The Car Met with the Accident near Murukatte, Honavar Uttara Kannada
around 12.15 AM (13th May 2023). There was one cow suddenly entered to
mid of the road and driver Mr Narayan couldn’t stop the car immediately and
the same time Maruti Omini came from opposite side and both the vehicle
met with the accident .
Mrs Mrs Saraswathi Y Bat was sitting in back seat along with her relative
Mrs Sharada G Bhat and she also got injured with the same accident. Entire
Incident is clearly explained in attached FIR Copy.

Due to accident, Mrs Saraswathi Y Bat got severely damaged with her both
the legs and right hand got severely fractured. she was immediately taken to
Govt Hospital Located in Honavar.
Then for further treatment she has shifted to Kasturba Hospital, Manipal,
Udupi.

She has undergone series of Surgery from last two months and currently she
is bedridden due to accident and taking the treatment from Kasturba
Hospital, Manipal, Udupi.

As per the doctor it may take 6 months to 1 year for the recovery depending
on her body conditions (Discharge Summary, Hospital Bills are attached)
