Subject :- Accident Information and Injury details.

Dear Sir/Madam,

As per the above subject, on 12th may 2023 I travelled to Yellapur, Karnataka
to attend one of relative’s marriage function. While coming back I met with
car accident (KA 47 M 9223) near Murukatte,Honavar around 12.15 am
(13 May 2023). Mr Narayan Ramakrishna Hegde was driving the car .

There was one cow suddenly entered to mid of the road and driver couldn’t
stop the car and the same time Maruti Omini came from opposite side and
both the vehicle met with the accident . I was sitting in back seat along with
my relative Mrs Sharada G Bhat and she also got injured with same accident.

Due to accident, I got severly damaged with my both the legs and right hand
got fractured . I was immediately taken to Govt Hospital Located in Honavar .
Then for further treatment I shifted to Kasturba Hospital,Manipal,Udupi .
Currently I am bedridden and taking treatment advised by the Doctor.
I do hereby declare that all the above information given by me are true to the
best of my knowledge and belief.
