Dear Sir/Madam:

Greetings!

I am PMSg Marvin Montuya, currently assigned at Talisay City Police Station,

Talisay City, Negros Occidental. I am writing you this letter to formally file my claims
against the vehicular accident that transpired last December 25, 2022 at around 4:40
in the afternoon involving a Ceres Bus Liner with plate no. NDD-3991 driven by Arvin
Lumanog y Sy, 47 years old, married, and a resident of Prk. Talisay, Brgy.
Castillano, Calatrava, Negros Occidental. I was onboard my Yamaha NMAX
motorcycle going north direction when I was hit by the Bus that was executing a turn
under the flyover located at Lacson St., Brgy. Banago, Bacolod City.

The incident was recorded at Station 3, Bacolod City Police Office. I have attached a
copy of the blotter incident report with this letter. We were scheduled to meet at the
police station on January 3, 2022 but I was surprised nobody turned up. Instead, I
was instructed to report to your office and I was given a list of requirements,
including this letter of claims.

Hence, the following is the breakdown of the expenses I incurred: 1) for the damage
of my Yamaha NMAX Motorcycle – Php 70,000.00; 2) for my medical expenses –
Php ____________; and 3) for compensation for the 15-days I was not able to work
– Php 27,000.00. I am asking for the total amount of ________________. All
receipts and the damage quote for the motorcycle are also attached with this letter.

It is with a heavy heart that I am writing you this letter but I am asking that you honor
your liability. Because of the incident, I missed work and other important
appointments. It happened on Christmas Day and the turmoil my family and I had to
go through to spend Christmas in a hospital and the inconvenience it brought me for
the following days can never be equated with money.

I am hoping that you look into this matter positively. I do not wish to do this but I
would gladly take this matter up to my lawyer should you fail to at least, provide me
the opportunity to air out my claims. You may reach me at my mobile no.
