process my affidavit of loss for my certificate of registration with the License No. N04-99-439436 and
claim it on my behalf. I am unable to accomplish this in person due to being out of the country for work
purposes.

The representative will show you a copy of my driver’s license as proof of my consent regarding the
matter. The bearer will also show her driver’s license ID for verification purposes.
