Dear OIC Madrazo:

This has reference to the Cash Surrender Value (CSV) and other claims in
behalf of my husband under policy number M-326129.

My late husband, LTC Mario L. Azarcon PA was a member of the

Philippine Army and regular member of the GSIS before he died on
February 18, 2004. Despite submission of all the requirements to the GSIS-
Pasay branch, I have not claimed any of the benefits due to me as
beneficiary.

In a letter dated October 23, 2007 by the Claims Division of the GSIS-Pasay,
a copy of which is hereto attached for your perusal, it states that that the
tentative computation of my husband’s CSV was not enough to cover his
accountabilities to the GSIS. In this regard, may I know what
accountabilities my husband had with the GSIS that barred me from
claiming the CSV of his policy?

As far as I know, my husband has no accountabilities with the GSIS. While

my husband had previously applied for a Salary loan, the check was never
claimed by him from the GSIS, thus it became stale. A certification dated
April 19, 2004 from the GSIS to that effect is hereto attached for your
perusal and consideration.

With my old age and being prone to Covid-19 virus, I can no longer
personally process, transact and go through the arduous process of
appearing before government offices and re-procure and re-submit
documentary requirements which I already had with your System. In this
regard, may I respectfully request your assistance to help me with my
claims and apprise me of whatever that needs to be done in order to claim
what is due to me.
